Description:
The CareSight Nurse Practitioner and Physician Assistants lead a care team that consists of Registered Nurses (RN) and Community Health Workers (CHW). They work closely with the patients’ primary care physician.
Position Essential Functions:
Provides in-home medical care to identify and treat health issues in collaboration with the patients’ primary care physicianLeads team that includes RNs and CHWsCollaborates with RNs and CHWs in the development of an integrated patient centered care planSupports patient and caregiver goalsCommunicates effectively with entire care team including the patients’ community primary care provider Travel requiredRequired:Must be fully COVID vaccinatedHolds active, unencumbered New York State licenseActive DEA licenseAt least two years of clinical experience delivering care to complex patients preferredActive, unrestricted New York State driver’s licenseOpenings in Monroe and Erie counties.
