We are currently seeking a motivated and experienced full-time Physical Therapist to join our team at a partnered Assisted Living Facility on the Upper East Side (NYC). All levels of experience are encouraged to apply and will train the right new graduate.

Provide 1:1 physical therapy treatment to residents within the facilityEvaluate patients and develop individualized treatment plans based on physician orders and patient needsDeliver care in accordance with professional standards and the established plan of careLead a weekly group exercise class focused on mobility and functional wellnessCollaborate with facility staff and caregivers to support patient outcomesPlan and implement therapy programs to improve or restore functional abilities, reduce limitations, and enhance quality of lifePerform and document initial evaluations, progress notes, and discharge summaries in a timely and accurate mannerMonitor patient progress and modify treatment plans as needed to achieve optimal resultsProvide care with a strong understanding of geriatric and memory care populations Physical Therapist Program GraduateLicensed Physical Therapist in the state of New York or eligible. The anticipated base salary range for this position is $90,000.00-$105,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
