Position Details: Position Status: Regular Part-time Location: Royal Park Shift: Days Shift Times: 0800-1615 Bi-weekly hours: 15.50 FTE:0.20 PCN:31RPSW003 Date closed: May 18, 2026 Rate of Pay: $ 38.56 - $51.27 The Opportunity: Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location Qualifications & Experience: Graduation from a recognized degree program in Social Work or equivalent program Registered with Alberta Association of Social Workers; Driver’s License Minimum of 2 years experience in a combination of clinical and continuing care roles; Gerontology experience is preferred
