Long-Term Care Pharmacist Kalamazoo Long Term Care | Kalamazoo, MI | Full-Time Pay: $124,800 – $133,120 per year (about $60–$64/hour)
About the Role Kalamazoo Long Term Care is looking for a pharmacist with long-term care experience to join our team. You’ll provide clinical pharmacy services built around the needs of LTC residents, keeping patients safe and making sure their therapy works, in line with our policies and all legal requirements. You’ll report to the Pharmacy Manager.
Schedule
• Pharmacy hours: Monday–Friday, 8:00 AM – 8:00 PM, and Saturday, 12:00 PM – 6:00 PM
• This position is part of a monthly on-call rotation
What You’ll Do
• Provide clinical pharmacy services tailored to long-term care residents
• Work closely with physicians, nursing staff, and the wider care team
• Conduct medication reviews and consultations to achieve the best therapeutic outcomes
• Dispense medications accurately and provide clinical oversight
• Direct pharmacy staff in mixing, packaging, labeling, and prescription order changes
• Ensure compliance with regulatory standards and LTC pharmacy best practices
• Document and communicate using electronic health record systems
• Provide excellent service to patients and caregivers
• Take part in team meetings focused on improving resident care
• Other duties as assigned
What You Bring
• PharmD from an accredited program
• Active Michigan pharmacist license (or eligibility)
• Long-term care pharmacy experience
• Clinical expertise with a focus on geriatrics
• Solid understanding of pharmaceutical regulations and guidelines
• Experience with pharmacy software; Framework experience preferred
• Strong analytical and problem-solving skills, with close attention to detail
• Excellent communication skills and the ability to work well with a multidisciplinary team
• Openness to others’ views and willingness to give and receive feedback
