Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

The Clinical Care Coordinator works under the direction of the CSP Team Leader and in cooperation with Mental Health Clinicians and/or the CSP Team to provide assessments, interventions, and support to Emotionally Disturbed or Chronically Mentally Ill adults and/or children. The Clinical Care Coordinator provides a safe and supportive environment for clients, completes functional assessments, plans for interventions, plans and organizes interdisciplinary team meetings, links clients with services, monitors client progress towards meeting identified individual goals, and functions as an advocate for client rights. The Clinical Care Coordinator provides clients with the training of critical skills identified as necessary for the individual to function in his/her living, working, learning, and social environments. The Clinical Care Coordinator shares on-call services with the CSP Case Management staff on a rotating basis.

Required Qualifications:
• Bachelor's Degree Prefer degree in behavioral health or education field (public, social services, health education, etc).
Preferred Qualifications:
• Experience working in a behavioral health.
