Job summary

We're looking for an experienced manager to lead a team dedicated to using a person-centred approach to make meaningful changes in people's lives. The Social Prescribing team focuses on building confidence, addressing various health factors, and connecting individuals to community resources to enhance their wellbeing and health.

As part of the Management Team at Southern Brooks, you'll help ensure the organisation runs smoothly and safely, keeping people and communities as our priority. You'll build strong relationships with other Southern Brooks teams, external partners, and community organisations across South Gloucestershire to deliver effective support to the community and individuals.

We are looking for someone with knowledge and practical expertise in empowering individuals to lead healthier lives, managing caseloads, overseeing projects, and supporting team development and service improvement.

This is a part-time role at 30 hours per week (to include Wednesdays). Based in our Patchway office - near to Cribbs Causeway and an easy commute from Bristol by bus, train, bike or car - with travel across South Gloucestershire including Southern Brooks offices in Kingswood and Patchway, and some working from home. Travel expenses over and above your usual commute will be reimbursed.

Main duties of the job

Lead and support the Social Prescribing, MINT Social Prescribing and DHI Community Connector services, ensuring high-quality, person-centred delivery.Provide effective line management, supervision and development for staff, supporting wellbeing, performance and positive team working.Oversee service quality, safeguarding, budgets, KPIs, contractual requirements, monitoring and reporting.Use data, feedback and learning to evaluate impact and continuously improve services.Build strong relationships with commissioners, funders, referral partners, community organisations and other stakeholders.Develop effective referral pathways, identify gaps in provision and work collaboratively to improve support for local people.Represent and positively promote the services at meetings, presentations and events.Contribute to Southern Brooks organisational strategy, planning and risk management as part of the Management Team.

Main purpose of job:

This role provides management and oversight across a range of teams and services, including Social Prescribing, the Mental Health & Wellbeing Integrated Network Team (MINT) and the DHI Community Connector service. These services support individuals to make sustainable changes in their lives through person-centred, preventative and non-medical approaches to health and wellbeing.

The main purpose of the role is to provide effective individual and team management, from recruitment and induction through to ongoing supervision, performance management and development. The role is also responsible for service development, including developing, monitoring and continuously improving services, learning from feedback, good practice and reflection.

The role also has responsibility for stakeholder and contract management, ensuring contractual deliverables and KPIs are met, quarterly reporting requirements are fulfilled, and there is regular communication with commissioners and other key stakeholders.

As part of the Management Team at Southern Brooks, you will contribute to a safe, well-run organisation that puts people and communities at the heart of everything it does. You will build strong relationships with teams across Southern Brooks, external partners and community organisations to support effective services for individuals and communities.

1. People Management & Leadership

Provide line management, supervision and support to employees across the identified projects, including the Social Prescribing Team Leader, Social Prescribing Link Workers and DHI Community Connector. Manage performance, attendance, development and wellbeing, addressing issues appropriately and supporting staff to perform effectively. Set the vision and direction for the team and support effective team working and service development. Work collaboratively with the team to develop and deliver an Operational Plan aligned with project targets, organisational priorities and service development needs. Contribute to organisational strategy, planning, risk management and wider Management Team activity. Deputise for the Head of Wellbeing & Health where required.2. Service & Contract Management

Oversee the day-to-day delivery and development of the Social Prescribing, MINT Social Prescribing and DHI Community Connector services, ensuring they are safe, effective, person-centred and meet agreed contractual requirements and outcomes. Monitor service and contract performance against agreed targets, KPIs and outcomes, identifying areas of underperformance and taking appropriate action. Ensure services meet agreed quality standards and organisational policies and procedures and continuously identify opportunities for improvement. Develop and maintain effective processes, systems and SOPs to support consistent service delivery and good practice. Ensure appropriate safeguarding processes are embedded across services, including recording, reporting and escalation in line with organisational policy and procedures. Ensure compliance with relevant governance requirements, including GDPR, health and safety, lone working and other organisational policies. Ensure effective recording, monitoring and reporting systems are in place, with accurate and timely information used to demonstrate impact and inform service improvement. Manage project budgets with support from senior managers, ensuring resources are used effectively and in line with contractual requirements. Build and maintain effective relationships with commissioners, funders, referral partners and other stakeholders, ensuring effective communication and partnership working. Develop and maintain effective referral pathways and identify gaps in service provision, working collaboratively with wider Southern Brooks teams to address these where possible. Represent and positively promote the services to external stakeholders through meetings, presentations and events.

Person Specification

Experience Essential

- Experience of managing teams including supervision, performance management and development. - Experience of managing projects including reporting and measuring outcomes. - Experience of supporting people with complex lives affecting their health and wellbeing, including working with diverse communities and people with a range of needs. - Understanding of mental health and barriers to accessing services, and experience of using approaches such as motivational interviewing, active listening, strengths-based, trauma-aware and person-centred practice. - Experience of partnership working, including building and maintaining relationships with a variety of stakeholders. - Experience of safeguarding adults and children, including recording and reporting in line with organisational policy and procedures. - Understanding of governance, compliance and good practice, including GDPR, employment, health and safety and lone working. - Experience of managing services to ensure quality, performance and agreed outcomes are achieved, in line with contractual and reporting requirements. - Experience of managing relationships with commissioners and other key stakeholders in relation to service delivery and performance.

Desirable

- Experience of managing project budgets - Experience of managing or running social prescribing, or similar health prevention and promotion programmes within VCSE organisations, and knowledge of social prescribing practice.

Job-related skills Essential

- Ability to plan, manage and deliver programmes or services effectively, identify improvements and implement change. - Ability to monitor and manage performance, analyse information and produce accurate reports to demonstrate outcomes and impact. - Excellent communication and relationship-building skills with diverse stakeholders. - Strong IT skills, including MS Office, databases and management information software. - Ability to manage own workload and prioritise effectively.

Qualifications Desirable

Degree or equivalent qualification in a relevant field, or equivalent relevant experience.

Working Conditions Essential

- Occasional evening and weekend work may be required - A driving licence or access to transport and willingness to travel across South Gloucestershire and occasionally work from home - DBS check required
