About this role

Job summary

The post holder will be responsible for assessing and determining eligibility for people with complex health needs who are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act. The service also commissions packages of care for clients requiring Transforming Care and Neurological Rehabilitation pathways.

The post holder will be responsible to clinically plan and commission the care required, ensuring that the care is safe, effective and good quality. Communication and relationships with individuals and independent providers is a key part of this role as well as abilities to manage risks, problem solve influence and negotiate to ensure appropriate clinical commissioning case management of complex individuals.

The post holder will chair multi-disciplinary meetings in order to make eligibility recommendations for NHS Continuing Healthcare and NHS Funded Nursing Care. Complete reviews of commissioned packages ensuring that people are receiving high quality safe and effective care as well as ensuring they still have eligibility for the relevant funding stream. You will also provide expert advice to other professionals commissioned to provide care in relation to NHS continuing Healthcare and NHS Funded Nursing Care packages.

Main duties of the job

The post holder will act as CHC commissioning nurse for patients whose care is either jointly or solely funded by NHS GM and the care is provided by the independent sector

Identify and case manage clients who may be eligible for NHS funding, chair and coordinate eligibility assessments and reviews

Provide expert advice to other professionals commissioned to provide care in relation to NHS Individualised Commissioning

Complete the restitution process for all claims which includes presentation of findings to the NHS GM multi-agency funding panel and the NHS England Independent review panel

Contribute to the quality monitoring of care home providers

Support with the duty function of the service

Promote ICSs vision and mission and to uphold ICSs values in all day to day activities and delivery of services.

Participate in the objective setting process as part of the annual Performance Development Review/appraisal process, to understand how own role and objectives are linked to team, directorate and corporate objectives, to review what aspects of your role are being done well, and to identify any areas for development.

Undertake relevant activities and mandatory training to meet objectives identified in Personal Development Plan.

Adhere to all ICS policies and guidelines, including HR, Information Governance, Risk Management and Health & Safety policies.

Comply with relevant ICS and own professional codes of conduct and accountability.

Maintain professional registration if this is a requirement of the job.

2. Reporting all incidents, near misses and hazards in line with ICSs reporting arrangements/system

3. Undertaking a statutory duty of care for your own personal safety and that of others

4. Attending all statutory and mandatory health and safety training, appropriate to the role.

Maintain the security and confidentiality of information you come across in your role in ICS in line with ICS policies and protocols. All staff have a duty to complete training on Information Governance as part of Mandatory Training and on an on-going basis through Essential Training updates.

All employees have a responsibility to protect and safeguard people (children and adults). They must be aware of child and adult safeguarding procedures and who to contact within ICS for further advice. All employees are required to attend safeguarding awareness training and to undertake additional training appropriate to their role.

PROFESSIONAL REGISTRATION Essential

Nursing qualification or equivalent Allied health professionals, Occupational Therapist, Physiotherapy etc

Qualifications Essential

BSC (Hons) Degree in Nursing or related subjects Nursing qualification or equivalent Allied health professionals, Occupational Therapist, Physiotherapy etc Or relevant experience

Experience Essential

Significant nursing experience at band 5 or equivalent experience Evidence of post qualifying and continuing professional development Experience of working as a Registered Nurse with patients with multiple/complex needs Responsibility for a defined caseload which includes assessment, planning, implementation and evaluation of nursing care Experience in working within a multi-disciplinary team setting Experience of clinical supervision and reflective practice discussions

Experience of working with Older People with Mental Health difficulties
