Job summary
The position requires a qualified pharmacist with an independent prescribing qualification or who has CPPE completion certificate, who will operate within their professional limits as a member of a multidisciplinary team that includes GPs, ANPs, Physician Associates, Paramedics, and First Contact Physiotherapists. You would be also required to support the delivery of the PCN contract requirements and associated targets.
Main duties of the job
Clinical & Patient Care
Conduct face-to-face clinical medication reviews for polypharmacy, elderly, and complex patients. Deliver a repeat prescription service, authorising repeat and acute prescriptions. Monitor medication adherence, support shared decision-making, and promote self-care. Set up long-term condition clinics (e.g., blood pressure control) and manage acute/minor ailments. Answer complex medicine enquiries from staff, patients, and community pharmacies.Medicines Optimisation & Governance
Support the delivery of the NWL Medicines Management Enhanced Service. Lead on prescribing projects (e.g., diabetes) and antimicrobial stewardship initiatives. Implement national prescribing guidelines and maintain a strong understanding of CQC standards. Promote safe medicines disposal, inhaler recycling, and waste reduction. Improve medicine safety and ordering systems within local care homes.Collaborative Working & Professionalism
Work with ICB pharmacists, PCN pharmacy teams, and wider health/social care networks. Provide clinical supervision for PCN pharmacy technicians where required. Attend practice learning, significant event meetings, and training with GP supervisors. Maintain continuous professional development and knowledge of practice clinical systems.

Hounslow Family Practice is a partnership of two GPs, providing care for 6,300 patients in Hounslow. Our exceptional clinical team includes 2 GP Partners, 2 Sessional GPs, a Clinical Pharmacist, 1 Practice Nurse, 1 ANP, and 2 Health Care Assistants. This clinical team is supported by an administrative staff, which includes a Practice Manager, an Administration team, and a Reception team, all dedicated to serving our patient population.

Patient-facing long-term condition clinics:Consult with patients who have one or more medical conditions that require medication optimization (e.g., COPD, asthma, hypertension).
Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments (i.e., medication optimization).
Provide GPs with recommendations for medication improvements.Conduct clinical medication reviews with patients and offer recommendations to the senior clinical pharmacist, nurses, and/or GP regarding prescribing and monitoring.Respond to medicine-related inquiries from GPs, practice staff, other healthcare teams (e.g., community pharmacy), and patients.Offer solutions and recommendations.Follow up with patients to monitor the effects of any changes.Review the use of medications commonly linked to unplanned hospital admissions and readmissions through audits and individual patient reviews.

Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines.

Monitor practice prescribing in accordance with the local formulary and recommend to GPs which medications should be prescribed by hospital doctors (red drugs) or be subject to shared care (amber drugs). Audit the practices compliance with NICE technology assessment guidance.

Person Specification
Qualifications Essential
Masters degree in pharmacy (MPharm) (or equivalent) Mandatory registration with General Pharmaceutical Council Independent prescriber or CPPE completion certificate Primary Care experience In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare Excellent written and verbal communication skills with patients and colleagues. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
