Job summary
We are looking for enthusiastic and patient-centred GPs to join our Primary Care Network (PCN) as soon as they are able, to work within our Proactive Care Team. The role with be focussed on helping to care for and support patients living with frailty and long term conditions in their own homes. There is support from an experience GP with an interest in frailty, and other AHPs with an interest and experience in frailty.
Additionally, we have flexibility for GPs to work within local GP practices over the winter period should they desire, providing additional sessions and capacity.

A Flexible Role Designed Around You
We recognise that one size does not fit all, so we are keen to develop a working pattern that suits the successful applicant(s). We are open to a variety of arrangements, including
Full-time or part-time hours.
School-hour working or Traditional full-day sessions.
The role would be based at the PCN Hub in Bridgwater and involving visiting patients in the community predominantly.
Our priority is to create a role that is manageable, enjoyable and sustainable.

What We're Looking For

Hold full GMC registration with a licence to practise.
Are included on the NHS England GP Performers List.
Have experience within Somerset
Are adaptable and keen to experience different practice environments.
