Victoria Primary Care Network is a high achieving, pro-active and ambitious PCN. The successful applicant will work in an exciting new role across the PCN's footprint as part of a wider multi-disciplinary team. This role will support all four practices (see organisation overview below) on a rota basis. We feel this role will have a positive impact on the network as our aim is to provide exemplary patient care, whilst finding innovative solutions in general practice to deliver the best care we can to our patients.

Working under the direct supervision of the Practice Nurses and strictly in accordance with specific practice guidelines and protocols, the GPA will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.

Working Hours

Saturdays - 9am to 5pm

Main duties of the job

Supporting GPs with basic observations as required e.g. taking blood pressure, ECGs and phlebotomy.

Process cholesterol blood results and perform Q risk score, arranging follow-up letters and/or appointments as required.

Non-diabetic hypoglycaemia processing results for NHS health checks, arranging follow up appointments and coding diagnosis.

Record Home monitoring / pharmacy blood pressure readings and follow and refer to the appropriate clinician.

Supporting with diary recall for blood tests.

Take part in a pilot for model working towards Frailty team within PCN to provide support to clinicians, reduce patient hospital admissions.

Complete annual health check reviews.

Support delivery of Routine immunisations, B12, Influenza, Pneumo, Shingles.

Chaperoning and assisting patients where appropriate who are being examined by another clinician.

Processing and management of laboratory samples requested by GPs/nurses.

Surgical equipment and vaccine re-stocking and stock rotation.

Clearing and re-stocking consulting rooms.

Person Specification

Qualifications Essential

Completion of HCA Care Certificate Competence in core skills, phlebotomy, blood pressure monitoring, ECG recording Ability to work within a team Strong communication and interpersonal skills

Desirable

Experience working as a Health Care Assistant in Primary Care Understanding of safeguarding and clinical governance IT proficiency and ability to maintain accurate clinical records Knowledge of GP EMIS systems Knowledge of DocMan Experience working as a Health Care Assistant in Primary Care Full UK Driving licence
