Main duties of the job

West Somerset PCN has an established central team based at our Living Better Hub in Williton and a strong track record of collaborative working across our practices. We are well developed in neighbourhood working, bringing together primary care, community health, social care and voluntary and community partners to develop services around the needs of our population.

This is a varied, hands-on role combining data analysis, digital systems, reporting, communications and service improvement. You will build and run clinical searches, develop reports and dashboards, support population health and proactive care work, and help us use our data to identify trends, gaps and opportunities for improvement.

You will also help our teams make effective use of digital technology, support the development of our digital maturity, contribute to projects and new ways of working, and provide practical support to staff. The role will also support communications and promotion of PCN services, programmes and events to practices, patients and wider partners.
