Job summary
The Ridge PCN are looking for an experienced general practice nurse to play a key role in supporting the health and wellbeing of residents living in care homes.
The PCN are looking for someone to commit to 30 hours per week.
Interviews will take place in early November.
Main duties of the job
Ridge Primary Care Network (PCN) is looking for a caring, compassionate Experienced General Practice Nurse with significant post-registration expertise in frailty. The Experienced General Practice Nurse will be focusing on delivering high-quality, coordinated care to residents in care homes across the Ridge locality. This will include providing care from initial history taking, clinical assessment, diagnosis, treatment, to an initial treatment plan and assistance in completing Respect forms with the care homes and ensuring they are available on Kent Medical Community Record [KMCR]. The post-holder work assists the nominated practice GP to ensure compliance with the PCN Care Home Direct Enhanced Service. They will also ensure patients within the care homes receive effective chronic disease management.

The Ridge Primary Care Network (PCN) is a semi-rural network established in 2019, serving a population of approximately 36,000 patients across four GP surgeries: Bearsted Medical Practice, Len Valley Practice, Sutton Valence Group Practice, and The Orchard Surgery in Langley. Our PCN has a strong record of collaboration and innovation, successfully delivering a range of services tailored to the needs of our local community. West Kent Primary Care is an innovative GP Federation whose shareholders include all fifty practices in West Kent, covering 500,000 patients. Within this, the practices are divided into 9 Primary Care Networks (PCN). We are made up of a core operations team, a PCN based workforce including clinical and non-clinical staff and a Board of Executive Directors (four local GPs and twelve PCN clinical directors). The federation also offers a variety of services including the Community Diabetes Service, and a Frailty Service.

Support public health and screening programmes, including immunisations, vaccinations, where appropriate.
Completing Respect forms.
Deliver expert nursing assessments and manage complex care needs of care home residents within Ridge PCN in partnership with GP.
Support patient and public involvement to enhance patient care and service delivery.
Create a trusting partnership with patients and/or relatives/carers to communicate and explain complex medical issues
Mentor, support, and supervise new-to-general-practice nurses, student nurses, and other healthcare professionals (e.g., nursing associates, HCSWs) across the PCN by agreement and when appropriate.

Qualifications Essential
Degree in Nursing or Registered Nurse Degree Apprenticeship Registered with Nursing and Midwifery Council More than one post-registration qualifications and CPD, for example certificates, diplomas or professional development courses at level 6 (e.g. long-term conditions care and/or public health initiatives) Nurse Prescriber
Experience Essential
Management of patients with long-term conditions Management of patients with complex needs Local plan and national health policy Understanding of a range of assessment methods.
Desirable
