This role is offered on a permanent basis. The hours are full time, 37.5 hours per week.
The Home First service is looking for a motivated Band 4 who will be responsible for supporting people recently discharged from hospital to maximise the person's potential for independence and work with health and social care services facilitating a case management approach with health and ongoing care and support needs.If you feel this is you and are looking for a new experience then keep reading.
The Neighbourhood Connector will act as a single point of contact for patients and their representatives. They will develop relationships that will enable them to work in a personalised way to help the person live their best life possible.
Main duties of the job
The post holder will support a defined group of individuals that will benefit from a case management/self-care approach to promote independence. To visit individuals at home to undertake personal assessments and ensure that every person is supported to live the life they wish, to the best of their abilities.
The Neighbourhood Connector will need to establish and maintain excellent communication with individuals and explore complex issues relating to care options and decisions and to sustain effective working relationships across all health and social care services.

The post holder will be responsible for developing excellent working knowledge of the support available from partner organisations in health, social care and the voluntary sector to encourage and guide this group of individuals to access these services where appropriate.
They will have the skills and that will enable them to support the person to navigate services and activities that are available to them in their community and make informed choices.
Person Specification
Qualifications Essential
NVQ Level 3 in health and social care management or similar qualification, or relevant experience GCSEs English & Maths Grade C or above

Health Coaching qualification
Experience Essential
Experience of working in a health and social care environment role Experience of working independently, unsupervised providing high quality services Have and understanding of the needs of people/adults with frailty or chronic health problems, particularly in relation to promoting their independence. Ability to work effectively alongside different service providers/patients/carers, supporting individuals to maximise self-management Able to communicate with patients in an empathic manner, adapting strategies to meet their needs To have a full driving licence and business insurance.

Experience or knowledge of health coaching and/or motivational interviewing techniques
