About this role

Job summary

We are looking to recruit a Young Persons Care Coordinator to work as part of our Social Prescribing Team. Social prescribing connects and signposts people to practical and emotional support such as peer support groups, activities, housing, finances and lots more. The Young Persons Care Coordinator will support our 5-25-year-old patients.

The working pattern will be: Monday 9am-5pm, Tuesday 9am-5pm, Wednesday 9am-4:30pm, Thursday 10am-6pm and Friday 9am to 5pm (total of 37 hours).

**We do not offer overseas sponsorship with this role.

Main duties of the job

Our ideal candidate would be committed to supporting the wellbeing of young people. A passion for safeguarding and promoting the welfare of children and young people is essential.

You will have experience working with young people aged 5 to 25 and their families, and a good understanding of how to support young peoples development and wellbeing.

Job description Job responsibilities

Help young people to manage their health and wellbeing needs, answering their queries, signposting and supporting them to access a variety of services that will help them.

Work as part of the Social Prescribing Team to provide personalised support and build positive relationships with young patients and their family.

Coordinate with other services and partners to plan and organise community events/services which benefit our patients such as Wellbeing Drop-ins and health events.

Ensure that young people have good quality information about a wide variety of available support options to help them make choices about their care.

Manage and plan own workload working with staff, partners and stakeholders to develop and support PCN/Practice projects which improve health and wellbeing.

Assist the Social Prescribing Team by triaging referrals to appropriate support within the team such as the internal safeguarding team

Attend safeguarding meetings where relevant or necessary and assisting the Practices safeguarding with ad hoc duties such as covering the safeguarding emails and internal tasks when safeguarding staff are not available.

Support Care Leavers and Young Carers by signposting to services that would help them.

Support our NEET patients (Not in Education, Employment or Training) with activities that help them such as CV writing, interview techniques and job searching.

Ability to identify risk and assess/manage risk when working with individuals and recognise the limits of their expertise which necessitate escalation/onward referral.

Assist patients to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.

Person Specification

Qualifications Essential

At a minimum, a Level 3 or above in Information, Advice and Guidance and/or Mentoring or Coaching, or equivalent experience

Personal Attributes Essential

Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential. Ability to organise, plan and prioritise on own initiative, including when under pressure. The ability to motivate people to make positive change. Ability to communicate effectively, both verbally and in writing, with people, their families, carers, partner agencies and stakeholders. Ability to identify risk and assess/manage risk when working with individuals and recognise the limits of their expertise which necessitate escalation/onward referral. Ability to maintain effective working relationships and to promote collaborative practice with colleagues. Demonstrates personal accountability, emotional resilience and works well under pressure.

Knowledge & Skills Essential

Experience of using word, excel and PowerPoint including ability to use word processing skills, emails and the internet to create simple plans and reports. Full UK driving license and access to own car.

Desirable

Knowledge of general practice clinical systems, such as, EMIS and SystmOne

Experience Essential

Experience in a similar role, working in a person-centred environment and working with young people aged 5 to 25 and their families. Experience of partnership/collaborative working and of building relationships across a variety of organisations. Some flexibility to work evenings and weekends may be required, as will the ability to travel to different locations at short notice

Desirable

Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work) Experience of working with or in general practice
