About this role

Job summary

Due to retirement and structural change, we are seeking an exceptional pharmacy technician to support the practice team, and patients.

What we are offering:

Main duties of the job

Prescribing Support: Assist ANPs & GPs with safe repeat prescribing systems, re-authorising prescriptions, and managing high-risk drug monitoring.

Medicines Reconciliation: Carry out medication reconciliation after hospital discharge and across other transitions of care.

Patient Liaison: Act as a direct point of contact for patient queries regarding prescriptions, dosage, and medication changes.

Multidisciplinary Teamwork: Work alongside GPs, ANPs, and nurses to improve patient outcomes.

Support health board prescribing incentive schemes, and clinical audits.

Conduct comprehensive medication reviews and optimising medication regimens for patients. Provide advice on medication management to patients, healthcare professionals, and the multidisciplinary team. Correspond with secondary teams if clarification needed of prescribing plans. Deal with all discharge medication reconciliation.Support health board prescribing incentive schemes and clinical audits.

Person Specification

Experience Desirable

Experience of working within a primary care environment.

Qualifications Essential

Certificate of Higher Education in Pharmacy Technician Practice (Level 4). Must be registered with The General Pharmaceutical Council (GPhC).

Desirable

Professional knowledge of medicines managements systems and process in an integrated NHS.
