As a Support Coordinator, you will play a vital role in empowering individuals with developmental disabilities to live full, self-directed lives. Through person-centered planning and ongoing coordination, you’ll help connect individuals and families to essential medical, social, and community resources while promoting independence, choice, and overall wellness. Conduct comprehensive assessments and ongoing reassessments to identify individual strengths, needs, and service opportunities Develop and update person-centered care plans that reflect individual goals, preferences, and progress Coordinate and connect individuals and families to appropriate services, supports, and community resources Monitor service delivery to ensure quality, effectiveness, and alignment with individual goals Maintain timely, accurate documentation in accordance with agency standards Collaborate with interdisciplinary teams to review progress, evaluate services, and recommend adjustments Travel to various locations to complete assessments and provide ongoing support
