Empower Recovery and Wellness – Join Us as a Community Health Worker (Behavioral Health Focus) Schedule: 8 am - 5 pm As a Community Health Worker in our Behavioral Health service line, you’ll play a crucial role in connecting individuals with mental health and/or substance use challenges to the support they need to thrive. Through outreach, advocacy, and education, you’ll bridge the gap between behavioral health services, social supports, and the community—helping clients build confidence, navigate complex systems, and work toward long-term wellness. Support patients in accessing mental health and substance use treatment services Provide outreach through home visits, phone calls, or community-based meetings Educate individuals and families on wellness, recovery, and behavioral health risk reduction Coordinate transportation and help manage appointments and follow-up care Assist with medication adherence and self-management strategies as directed by clinical staff Navigate social service systems, including MO HealthNet, DSS, and local community resources Build relationships with care teams and community partners to reduce health disparities Conduct PRAPARE assessments and document patient interactions in the electronic health record Participate in team meetings and follow care coordination guidelines as outlined by MO HealthNet
