NELFT are seeking enthusiastic and highly motivated Registered Nurses to complement our skill mix within the CHCS across Havering. The Community Nurse will support the provision of holistic health care, as delegated within the agreed level of competency, to patients and families/carers in their own homes, including residential care homes, and in nurse-led clinics.
Main duties of the job We are looking for an experienced and motivated Band 7 District Nurse Team Leader, to join our Community Nursing Team in Romford Clinic.
As a band 7 district nurse team leader you will manage a community nursing team and hold accountability for a complex patient caseload. You will provide clinical leadership, co-ordinate multidisciplinary care and act as a specialist resource for patients in their own homes.

Main Duties
Clinical Responsibilities
Manage day to day operations and workflows of the community nursing teams. Conduct line management, staff appraisals, clinical supervisions and sickness absence management. Delegate tasks appropriately to nurses, nurse associates and health care assistants Be accountable for a specialised, highly complex caseload Participate in clinical audits, complete timely audits for service, ensure medical devices are serviced, logged in and out. Track any misplaced equipment. Undertake holistic assessments of patients with complex health needs in community settings Plan, implement, and evaluate care, ensuring safe and effective clinical practice Deliver a range of nursing interventions including wound care, long-term condition management, and end-of-life care Support hospital discharge and admission avoidance pathways Caseload & Care Coordination
Manage and prioritise a defined caseload, ensuring high standards of care delivery Act as a key worker, coordinating care across health and social care services Monitor and review care plans, adapting interventions as required Leadership & Team Support
Contribute to daily team coordination and workload allocation Support the Team Leader in service delivery and development Multidisciplinary Working
Work closely with GPs, therapists, social services, and other agencies Participate in integrated team meetings and joint care planning Build strong relationships with patients, families, and carers Quality & Governance
Maintain accurate and timely clinical documentation Adhere to Trust policies, clinical standards, and professional guidelines Participate in audits, service improvement, and quality initiatives
Applicants will have NMC Registration and demonstrate the ability to innovate and motivate. Applicants are required to evidence a full UK driving licence and the ability to travel to a variety of sites (This criteria will be reasonably adjusted to meet the requirements of a disabled applicant). This position involves working weekends & some bank holidays.
Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.
Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement – Outer London This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
