The post holder will have successfully completed an NVQ in Health and Social Care or equivalent and will work under the supervision of the Integrated Discharge Hub Lead. Extensive experience in the NHS and Social Care delivery is essential.

The post holder will work in partnership with NELFT community services, Waltham Forest Social Care and other agencies to provide short term, responsive and quality homebased care and rehabilitation for service users.

Main duties of the job Using a reablement approach the post holder will facilitate service user independence and empowerment as far as possible with regard to existing medical conditions and disabilities and assist the service user to learn or regain skills with personal and domestic tasks as part of their therapeutic plan. To give holistic care to service users in their own home. To plan and organise Bridging Service daily rosters, coordinating the shift to ensure all visits are completed and staffs safety is maintained. To brief Team Lead/coordinators on all relevant developments for each service user. To participate in a duty rota to provide care for the service users to include unsociable hours and weekends. To assist in the administration of medication, if necessary, with suitable training. To have knowledge relating to Trust protocols and procedures and adhere to them, particularly manual handling. To be responsible for providing accurate records of information required by the Trust for audit purposes. To maintain accurate records in the service users own home or elsewhere. Identify and report any changes in service user care needs or changes in physical or emotional well-being to the Team Lead/coordinators.

The purpose of this job description is to outline the level of responsibility and accountability of this post. This will ensure that all work undertaken by our staff is clearly identified and carried out under clear lines of accountability.

The post holder will work in partnership with NELFT community services, Waltham Forest Social Care and other agencies to provide short term, responsive and quality homebased care and rehabilitation for service users.

The post holder will be responsible for the operational management of The Bridging Care and Rehabilitation Service and to ensure care is delivered in line with agreed care plans. This will include staff management and rota coordination to ensure safe delivery of the service 7 days a week between 0700 to 2100 hours.

The service offers short-term homebased care and rehabilitation with the aim to:

Facilitate timely safe discharge from acute hospital care

Help prevent unnecessary admissions to care homes

Help prevent unnecessary hospital admissions

Help to regain confidence with living skills or support with long term care needs until such time a Social Services / private package of care can commence.

Promote a reablement and rehabilitation approach to care to enable patient to regain a level of independence.

Key Responsibilities:

Provide information to assist local resolution and complaints when required.

To be skilled in communication, establishing and maintaining good relationship with

service users, carers, members of the wider health and social care team or other agencies.

To maintain, update and develop personal, professional knowledge and skills achieving

high standards of service users care at all times. Reflect on practice regularly and plan personal development in order to achieve growth development. Access supervision from the Team Lead/coordinators and other staff to enhance own practice.

To maintain confidentiality in all dealings with service users and their records.

To support service users, relatives and other care imparting knowledge to promote positive health and safe use of available equipment.

Clinical Skills

To provide service users and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life.

To work alone in the community, always with access to support and supervision.

To maintain statistical returns as required by the Organisation, including entering clinical records onto the RIO system within the required time frame.

To attend and contribute to team meetings.

To report and document any incident or accident occurring to a service user or a member

of staff.

To have an understanding and implementation of safe moving and handling, promoting health and safety for service users.

To be responsible for the care and safe keeping of equipment issued and to report any

defect or loss.

To carry out delegated tasks as necessary to support the work of the team as instructed

by the Team Leader and other team members.

Contribute to the effective and efficient use of resources by ensuring that supplies are

available, alerting senior staff when supplies are short and by making suggestion to

reduce waste.

To maintain appropriate and up-to-date knowledge and skills, including mandatory training and to undertake continuing education in accordance with personal and service needs within the framework of a Personal Development Plan (PDP).

To be aware of and report any safeguarding issues and complete any documentation to

support this.

This is a description of the duties of the post as it is at present. This is not intended to

The job will be reviewed on a regular basis in order to ensure that the duties meet the requirements of the service and to make any changes necessary, keeping with service development and changing roles expected of this post. service users care.

Responsibility for ensuring that appropriate methods are initiated to facilitate effective communication with service users and informal carers who may experience communication difficulties e.g. those who have mental health issues, learning difficulties, cognitive or sensory impairment or those who require interpreters.

Have the ability to be patient and sensitively assist service users to maintain their independence.

Responsibility to maintain confidential, legible and accurate records.

Be able to use computer systems for effective communication of information.

Training

To ensure own continued professional development and support a culture of lifelong learning.

To undertake, and assist, in the planning of own mandatory training and workshops.

To undertake an annual appraisal, developing a personal development plan that includes clinical competencies reflecting the health needs of the local population and relates to Trust strategy.

Person Specification

Qualifications Essential

Health and Social Care training. e.g. NVQ level 3 or equivalent

Demonstrates a commitment to further learning & development

Care certificate qualification

Previous experience of leading a team and supervising staff
