About this role

Job summary

An exciting opportunity has arisen for an experienced and motivated Registered Nurse to join the East Lancashire District Nursing Service as a Senior Nurse within the Clinical Triage Hub. This permanent Band 6 role offers an excellent opportunity for a skilled District Nurse to further develop their clinical leadership, decision-making and triage expertise within a supportive and team.

The Clinical Triage Hub is central to ensuring patients receive timely, safe and effective care across the community. As a Senior Nurse, you will play a role in assessing and prioritising referrals, supporting the coordination of community nursing services. Using your District Nursing experience, you will make autonomous clinical decisions that help ensure patients receive the right care, at the right time.

Working closely with the Clinical Team Leader and multidisciplinary partners, you will contribute to service development. You will also mentor students and new staff, act as a role model to promote excellence in patient care.

We are seeking an enthusiastic and compassionate nurse with excellent communication, organisational and clinical assessment skills who is committed to delivering high-quality, patient-centred care. The role requires flexibility to work across a seven-day service, including rotational day and evening shifts, weekends and Bank Holidays. Main duties of the job

As a Senior Nurse within the Clinical Triage Hub, you will play a key role in delivering safe, effective and responsive community care. You will undertake clinical triage and assessment of referrals, ensuring patients receive the right care at the right time whilst supporting clinical decision-making across the District Nursing Service.

Working closely with the Clinical Team Leader and wider multidisciplinary teams, you will provide professional leadership, support workforce planning and staff allocation, and contribute to service development. You will also mentor students and new team members, promoting a culture of learning, excellence and continuous improvement.

We are looking for an experienced and enthusiastic nurse with strong clinical assessment, communication and leadership skills, who is passionate about delivering high-quality patient care and supporting colleagues to achieve the best outcomes for patients.

Person Specification

Qualifications Essential

Registered Nurse i.e. RN Adult branch Community Specialist Practitioner Qualification or willingness to work towards this

Desirable

Non-medical prescriber V100/V150/V300 Leadership training

Experience Essential

Substantial district nursing experience 12 Months Experience in holistic assessment of complex patients and care coordination Experience of multidisciplinary team working

Desirable

Experience of caseload management and work allocation

Knowledge and Skills Essential

Ability to develop effective working relations with other agencies and professionals Ability to manage own and others workload and delegate effectively
