Job summary
Yorkshire Health Network (YHN) is seeking an experienced General Practitioner (GP) to work 12 hours per week, leading the Proactive Frailty Team (PAFS) and delivering comprehensive, high-quality medical care to patients referred to the service.
Main duties of the job
The Lead General Practitioner: Proactive Frailty Team is responsible for medically leading the multi-disciplinary team during the hours that the service is operational. They will be required to provide support and advice to other clinical and non-clinical colleagues as required and to work with the Operational Managers and Clinical Lead on the delivery of the service, escalating issues as required.

Make professional, autonomous decisions in relation to presenting problems and assessment outcomes.Assess the health care needs of patients with undifferentiated and undiagnosed problems.To record clear and contemporaneous notes to agreed standards using clinical systems and communicating appropriately to community partners including GP practices for follow up.Act as the GP Lead for the PAFS multi-disciplinary team, providing guidance and support to team members.Request investigations as required.Provide health education as required.Collect data for audit purposes and comply with QOF requirements.Compile and issue computer-generated acute prescriptions.To work within the prescribing formulary.To be responsible for personal time management.Work effectively within a multidisciplinary team and the management team to maximise effective and efficient delivery of care to patients.To promote close teamwork between colleagues and continually strive to improve the service.To regularly update clinical skills and knowledge.Maintain professional registration and licence to practice.
Person Specification
Experience Essential
Supporting and mentoring team members and junior staff. Working effectively within a multidisciplinary team. Maintaining accurate and timely patient medical records. Adhering to clinical governance frameworks and quality improvement initiatives. Understanding the role of medical services in a wider public health agenda.
Skills Essential
Highly developed interpersonal, communication and relationship skills. Clinical assessment and diagnostic skills. Compassionate and patient centered approach. Ability to work as a team member and autonomously.
Qualifications Essential
Medical Degree. GMC registration with no restrictions on license to practise or pending investigations. Registered on the GP Performers List. Certificate of Completion of Training (CCT) in General Practice. Enhanced training in frailty or care of the elderly.
