About this role

Job summary

Amersham Health Centre is looking to recruit an enthusiastic and committed Salaried GP, with a view to Partnership, to join our friendly and supportive team.

This is an excellent opportunity for a GP who is looking for a long-term role within a well-established practice and who may be interested in progressing to Partnership in the future.

The post is for 6 sessions per week, ideally working Tuesday, Wednesday and Friday. We would, however, be happy to discuss some flexibility around the working pattern for the right candidate.

We are looking for a GP who values continuity of care, enjoys working as part of a multidisciplinary team and is keen to contribute to the ongoing development of the practice.

We offer a supportive working environment, 15-minute appointments, an experienced clinical and administrative team and a strong focus on providing high-quality patient care while maintaining a sustainable workload for our clinicians.

Main duties of the job

The successful candidate will provide the full range of general medical services expected within a busy GP practice, working collaboratively with our GP Partners, salaried GPs, nursing team and wider multidisciplinary team.

Key responsibilities will include:

Undertaking face-to-face and telephone consultations.

Managing acute and chronic medical conditions.

Reviewing and actioning pathology and investigation results.

Prescribing and medication reviews in accordance with current guidance.

Making appropriate referrals to secondary care and community services.

Participating in the practice's same-day and total triage system.

Providing continuity of care for patients with complex or ongoing health needs.

Contributing to QOF, enhanced services and other practice clinical targets.

Maintaining accurate and contemporaneous clinical records.

Participating in clinical meetings, significant event reviews and quality improvement activities.

Supporting safeguarding, clinical governance and patient safety processes.

Working collaboratively with the wider primary care team including pharmacists, physiotherapists, mental health practitioners and other PCN colleagues.

Contributing to the development of services within the practice.

For a candidate interested in progressing to Partnership, there will be the opportunity to become increasingly involved in the strategic, clinical and operational development of the practice.

About us

Amersham Health Centre is a well-established GP practice based in Amersham, Buckinghamshire, caring for approximately 14,000 patients.

We have a friendly and experienced team of GP Partners, salaried GPs, nurses, healthcare professionals, reception and administrative staff. We also work closely with a range of Primary Care Network professionals including pharmacists, pharmacy technicians, physiotherapists and mental health practitioners.

The practice is part of Mid Chiltern PCN and benefits from strong working relationships with neighbouring practices and community services.

We operate a total triage model, supported by AccuRx, to ensure patients are directed to the most appropriate clinician or service. We are continually looking at ways to improve access, efficiency and the experience of both patients and staff.

We are a forward-thinking practice with an emphasis on teamwork, continuity of care and maintaining a supportive working environment. The practice has experience of GP training and education and encourages professional development.

Job summaryThe postholder will work as an integral member of the clinical team, providing high-quality, patient-centred general medical services to the registered practice population.

The postholder will manage a varied clinical workload, including acute and chronic disease management, preventative care, prescribing, referrals and follow-up.

The role requires effective communication and close working with the practice's multidisciplinary team and external healthcare providers.

The postholder will be expected to contribute positively to the development of the practice and participate in clinical governance, quality improvement and practice meetings.

Clinical responsibilitiesThe postholder will:

Provide GP consultations in accordance with the agreed clinical rota.

Assess, diagnose and manage patients presenting with acute and chronic medical conditions.

Undertake appropriate clinical examinations and investigations.

Develop appropriate management plans in partnership with patients.

Prescribe safely, effectively and in accordance with national and local guidance.

Review medication and respond appropriately to prescribing queries.

Review and action laboratory, radiology and other investigation results.

Make appropriate referrals to secondary care, community and other specialist services.

Provide appropriate follow-up and continuity of care.

Participate in the management of long-term conditions.

Support health promotion and disease prevention.

Provide appropriate advice regarding self-care and use of NHS services.

Complete clinical correspondence, medical reports and other documentation as required.

Participate in the practice's same-day and total triage arrangements.

Contribute to the management of urgent clinical presentations when required.
