Conexus Healthcare CIC is the community interest company and federation for general practice in Wakefield. We are setting up WorkWell, a new service that helps people with health conditions to return to work or stay in work. We have three Work and Health Coach roles available and are looking for people to join the team from the start.
About you
We care most about your people skills. You do not need a formal qualification, but you will have experience of Social Prescribing, coaching, advising, guiding or supporting people one to one. This could be in career advice, health or mental health support, employability programmes or mentoring.
You will be empathetic, confident talking about health, and able to build rapport with people from different backgrounds. You will be organised, comfortable using IT systems and able to keep clear records. We will give you the training you need and ongoing development.
Where you will work
Your base will be Sandal Castle Medical Centre, and you will work flexibly across the Wakefield district. You will need a full UK driving licence and business insurance, or another way to travel for work. We will pay reasonable travel expenses.

About the role
You will give one-to-one, personalised support to people across the Wakefield district. You will carry out a work and health assessment with each person and agree a clear action plan together.
You will offer career and employment advice, help people identify workplace adjustments, and connect them to local health, wellbeing and community services. You will manage your own caseload and work closely with GP practices, employers, jobcentres and voluntary groups.
Support is delivered face to face, online and by phone, from a mix of home working, local hubs and community venues.

Job description Job responsibilities Job Purpose
The Work and Health Coach gives one-to-one, personalised support to people in Wakefield who join the WorkWell programme. The role helps participants take practical steps to return to work, or to stay in work, while managing a physical or mental health condition.
You will carry out work and health assessments and agree a clear action plan with each participant. You will offer career and employment guidance, help identify workplace adjustments, and refer or signpost people to health, wellbeing and community services.
You will manage your own caseload and keep accurate, up-to-date records so the service can meet its targets and report on the difference it makes. You will build good working relationships with GP practices, employers, jobcentres and local voluntary and community groups.
Support is delivered face to face, online and by phone, from a mix of home working, local hubs and community venues across the Wakefield district.

Qualifications Essential
No formal qualifications are required. Willing to complete a Disclosure and Barring Service (DBS) check. Willing to complete the Personalised Care Institute accredited training for Health and Wellbeing Coaches /Social Prescribing Level 3 and mandatory Conexus Healthcare training.
