About this role

Job summary

The Enhanced Access Clinical Pharmacist provides safe, evidence-based and patient-centred clinical care within a multidisciplinary primary care service.

The role provides episodic care rather than continuity of care, assessing and managing patients based on their presenting need. This includes medication reviews, medicines-related queries, acute conditions and prescribing where appropriate. The post holder will ensure accurate documentation, safety-netting and clear handover to the patient's registered practice where ongoing care is required.

Independent Prescribing qualification is desirable.

Main duties of the job

Episodic care rather than continuity of care the pharmacist manages the patient's presenting clinical need during the Enhanced Access consultation.

Safe prescribing prescribing decisions must be based on sufficient clinical information and remain within the pharmacist's competence and the scope of the service.

Clear ownership where ongoing monitoring, treatment or review is required, responsibility must be clearly transferred to the patient's registered practice or appropriate clinician.

Effective clinical handover every consultation requiring further action must leave an accurate and actionable clinical record.

Safety-netting patients must understand what to do if their condition does not improve, deteriorates or new symptoms develop.

Appropriate escalation pharmacists must recognise when a patient requires GP, specialist, urgent or emergency care.

No inappropriate creation of continuity the pharmacist should not inadvertently become responsible for ongoing management of a patient where this sits outside the Enhanced Access service model.

Patient-centred care despite the episodic nature of the service, every patient should receive a safe, respectful and clinically appropriate consultation.

Prescribing in accordance with the NWL prescribing formulary and NICE guidance.

Job description Job responsibilities

Conduct face-to-face, telephone and remote clinical consultations. Assess and manage acute, minor and common conditions within scope of practice. Undertake medication reviews, medicines reconciliation and medicines optimisation. Process appropriate acute and repeat prescription requests in line with service protocols. Provide evidence-based medicines advice and support antimicrobial stewardship. Prescribe within competence and service protocols; independent prescribing is desirable. Identify red flags, safeguarding concerns and patients requiring escalation or onward referral. Provide clear safety-netting and ensure appropriate follow-up arrangements. Maintain accurate, contemporaneous clinical records and communicate relevant information to the patient's registered practice. Work collaboratively with GPs, nurses, HCAs, physiotherapists, pharmacists and other healthcare professionalsContribute to clinical governance, audit, quality improvement and medicines safetyMaintain professional registration, competence and CPD in accordance with GPhC requirements.

Person Specification

Qualifications Essential

Essential: Full registration with the General Pharmaceutical Council (GPhC) with no restrictions on practice. Appropriate primary care/community pharmacy experience. Up to date with mandatory training. Appropriate professional indemnity insurance in place Current enhanced DBS clearance as required for the role.

Desirable

Desirable: Independent Prescribing qualification. Experience of working within local primary care practices.

Experience Essential

Essential: Proven experience delivering high-quality NHS primary care pharmacy consultations. Experience undertaking medication reviews and medicines optimisation. Proficiency with SystmOne and relevant clinical systems, including ICE where required. Strong clinical, consultation, communication and time-management skills. Ability to work autonomously and prioritise workload under pressure. Ability to work effectively as part of a multidisciplinary team.

Desirable

Desirable: Experience working within local GP practices or a Primary Care Network. Experience of independent prescribing. Experience contributing to clinical audit, quality improvement and medicines safety.
