Job summary
The Hull Integrated Care Team for Older People (HICTOP) are looking for an experienced and passionate social worker with a keen interest in supporting the older adult population who have mental ill health or dementia.
We provide a service for people who have functional mental health needs (such as Depression and Anxiety) or organic needs (such as Dementia). We work together with public, private and voluntary services to provide the best possible care. Clients are seen in clinics, in their own homes, residential or other care settings by psychiatrists, community mental health nurses, specialist social workers, occupational therapists and support workers. We also provide support and advice for carers and family members.
The role is to provide a mental health social work service to those in need of support and interventions within a multi-disciplinary setting, in partnership with a range of other professionals.
The post holder will be responsible for undertaking person-centred assessments, outcome-based care planning and reviews in line with the the Care Act 2015.
The post holder will undertake direct work to support service users and carers and apply their knowledge and skills in respect of safeguarding the Mental health Act 2007 and the Mental Capacity Act 2005.
The role is key in ensuring that other social workers are professionally led through the provision of high quality social work supervision, critical reflection, and analysis.
Main duties of the job
The post holder will hold a caseload of service users with a variety of mental health needs, completing mental health assessments, risk assessments, care plans and CPA reviews. The post holder will work to the local authorities' operating model to arrange any packages of care which may be required and will work closely with colleagues in other Trust services such as in-patient as well as other partners and providers of care. The Post holder will work with service users in a range of settings and will work with them to agree what outcomes they hope to achieve. This will be through assessment of need using strength based and human rights approaches with an emphasis on prevention and independence. The post holder will contribute to a multidisciplinary team, providing specialist social work knowledge and supporting colleagues n an MDT forum.

Qualifications and Knowledge Essential
Professional knowledge gained through Diploma in Social Work or equivalent plus consolidation through period of indentured/supported practice Degree and current professional registration held with Social Work England. Post registration/graduate diploma or equivalent experience in related work area, to demonstrate specialist level of practice. Demonstrable experience of mentoring. Full understanding of relevant policy, legislation, drivers and their application to the service area, e.g. Safeguarding, Mental Health Act, Care Act, Mental Capacity Act, Social Inclusion, Liberty Protection Safeguards and other relevant legislation. Full understanding/application of relevant practice/standards/audit within identified service area. Standard IT skills
