About this role

Job summary

Join our friendly and supportive Integrated Care Teams based at Havant Health Centre, St Mary's campus, Waterlooville and Petersfield providing high-quality community-based care to patients registered with local GP practices. Working as part of a multi-disciplinary team, you will support registered practitioners by undertaking delegated nursing tasks and delivering person-centred care in patients' homes and community settings.

This rewarding role involves home visiting and lone working, helping to prevent unnecessary hospital admissions, support early discharge from hospital, and promote independence for patients living with long-term conditions and older adults with mental health needs. You will play a key role in improving patient outcomes and enhancing quality of life while working within a collaborative and compassionate healthcare team.

This is an excellent opportunity for a caring, motivated individual who enjoys working independently and making a real difference in their local community. Due to the community-based nature of this role, a full UK driving licence and access to a vehicle are essential to enable travel between patient visits across the locality.

There are also fixed term options available.

Main duties of the job

To provide a supporting role to registered practitioners in the Integrated

Community Team in the delivery of nursing care respecting patients' privacy and dignity at all times.

To act as key worker for patients under the direction of the registered practitioners.

To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.

To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.

To be responsible for progressing patient's treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient's progress within the team.

To perform a range of clinical nursing skills as directed by the registered practitioners:

*Skin care/Pressure Ulcer prevention

*Vital Signs - Temp/Pulse/Lying and Standing BP/Respiratory Rate

*Mouth care/Eye care/Toe nails and Finger nails

*Catheter care/Bladder washouts/Stoma care

*Catheterisation (under registered practitioner discretion -- NVQ III Training)

*Continence re-assessment

*Bowel care (suppositories)

*Venepuncture

*Basic Wound care

*Simple bandaging and paste bandaging

*Short stretch bandaging under supervision and after training

To provide information and advice to patients and carers on how to optimise functional ability, nutrition, health and safety, and health promotion.

Person Specification

Qualifications Essential

Relevant experience as a care assistant in a health care environment. Evidence of delivering care delegated from a range of registered professionals, in particular nursing. Evidence of ability to implement prescribed programmes of care for a range of conditions. NVQ 3 in health care related subject or willingness to undertake
