Job summary Package Description: As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This is a rewarding opportunity to deliver care where it matters most, in peoples homes and local clinics, helping individuals stay independent and supported within their own communities. This role is ideal for someone who enjoys working out in the community rather than being based in one location all day. You will build meaningful relationships with patients and play a key role in supporting their health and wellbeing in a more personal, one to one setting. This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:
Supporting patients with delegated nursing care in their own homes Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off Supporting holistic, evidence based care that meets each patients individual needs Maintaining accurate patient records using electronic systems Assisting with administrative tasks to support the team Planning and organizing your own daily visits across the communityThis role requires travel across the local area, so a UK driving license and access to a car is essential.

NVQ Level 2 or 3 in Health and Social Care, or equivalent experience A genuine interest in community based care and supporting patients in their own homes Full UK driving license and access to a car Good communication and interpersonal skills Ability to work independently and manage your own workload Basic IT skills including Microsoft OfficeDesirable
