Job summary
We are seeking a motivated and compassionate Practice Nurse to join our friendly and supportive practice. The successful candidate will provide high-quality nursing care to patients, including chronic disease management, health promotion, cervical screening, immunisations, wound care, and routine nursing treatments.
Working collaboratively with GPs and the wider multidisciplinary team, you will play a key role in delivering safe, patient-centred care and improving health outcomes within our practice. Main duties of the job
The Practice Nurse will deliver high-quality nursing care within a primary care setting, including chronic disease management, health promotion, immunisations, cervical screening, wound care, and routine treatment room services. The role involves supporting patients with long-term conditions, providing health education and advice, undertaking clinical assessments, maintaining accurate patient records, and working collaboratively with GPs and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.

Qualifications Essential
- Valid NMC registration (Nursing and Midwifery Council) - Experience in chronic disease management and diabetes care - Experience working in general practice or primary care
Desirable
- Post-registration time: A minimum of 1 to 2 years working as a registered nurse - Immunisations and vaccinations: In-date training for adult and childhood immunisations - Cytology competency: Training and experience performing cervical smear tests - Wound management: Advanced tissue viability and complex dressing skills - Prescribing support: Experience working with Patient Group Directives (PGDs) - Clinical software: Familiarity with primary care IT systems like EMIS Web - Audit and governance: Understanding of clinical audit and quality improvement processes
