Job summary
The post holder will provide specialist assessments and interventions to patients within a care group; participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice; liaise with other organisations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group. Have an understanding of psychosocial interventions to include psycho-education, wellness recovery action plans and discharge planning.
The post holder will lead the multi disciplinary team , working closely withother professionals and managers in the NMHT in the SOAMHS speciality.
Main duties of the job
As part of our community transformation plans we continuing to develop our Neighbourhood Mental Health Teams. For Specialist Older Adult services there are five teams across East Sussex with a range of multi disciplinary professionals in each team who will provide assessment, treatment and interventions including the provision of intensive support and treatment to enable people to receive care and treatment in their home setting and avoid the need for hospital admissions.
We are looking for a Band 7 Team Lead to provide Leadership and management to a MDT. This will include Leadership and clinical input of intensive support to older people in their home setting in the Hastings & Rother area. Person Specification
Qualifications Essential
Registered Nurse / Occupational Therapist with current registration (mental health / LD/ Adult) appropriate to the job role. Degree/diploma level supplemented by specialist qualification, training, experience, courses to masters level equivalent relevant to the care group Evidence of post registration training in relevant service area Completion of Mentorship Course/ENB equivalent Extensive post qualifying experience in relevant service area Experience of supervising staff and students Able to manage a caseload of patients with complex mental health needs Comprehensive knowledge of interventions specific to service area Awareness of Trust policies and service specification
