Job summary
We have an exciting new opportunity to join our team as a Practice Pharmacist.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.
The post holder will act within their professional boundaries, supporting and working alongside our medicines management team. In this role, you will be supported by our Practice Pharmacist who will manage and mentor them.
As part of a multi-disciplinary team, the post holder will take responsibility for areas of chronic disease management and long term conditions management within the practice, undertaking clinical medicines reviews and address public health and social needs of our patients.
Main duties of the job
Provide expertise in clinical medicines review & address public health & social needs of patients in GP practices
Reduce inappropriate polypharmacy & wasteful prescribing
Reconcile medicines following hospital discharge & work with patients & community pharmacists to ensure patients receive the medicines they need post discharge
Contribute to reductions in medicine related hospital admissions & readmissions by supporting patients to get the best outcomes from their medicines, identifying & addressing medicines related issues
Interface with community & hospital pharmacy colleagues & develop referral processes between primary care professionals including the promotion of the repeat dispensing service
Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions & medicines reaching review dates
Manage patients & health care professionals medicine queries
Increase quality & safety of prescribing through mechanisms such as audit & PDSA cycles
Manage practice formularies to improve the quality, safety, & cost effectiveness of prescribing
Implement drug withdrawals & alerts eg, MHRA aimed at improving medicines safety
Work with primary care professional & patients to implement NICE & other evidence-based guidelines
Contribute to multimorbidity reviews & MDT reviews
Provide medicines information & training to Practice healthcare professionals & admin staff. Review Pathology results for patients on known medicines.
Act as a source of medicines information

Qualifications Essential
Mandatory registration with General Pharmaceutical Council Independent Prescribing Qualification In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients) Able to plan, manage, monitor, advise and review general medicine optimization issues in core areas for long term conditions. Good IT Skills Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal patter and can refer to seniors or GPs when appropriate Able to work under pressure and meet deadlines Produce timely and informative reports Gain acceptance for recommendations and influence/ motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers Work effectively independently and as a team member Self-Motivated and adaptable Excellent patient manner and interpersonal skills High level of personal integrity Willingness to change practice in the light of experience Possess a professional appearance and manner To have drive, enthusiasm and initiative Flexible approach Ability to challenge constructively and able to work under pressure Full Driving License*
Desirable
Masters degree in Pharmacy (MPharm) or equivalent
