About this role

Job summary

An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner (ANP) to join Fenland Primary Care Network (PCN) delivering a high quality service for patients and care home residents across our four member practices.

The role focuses on providing autonomous, high-quality care including holistic enhanced assessments, management of acute and long-term conditions, medication reviews, prescribing, holding surgery clinics and potential for some mobile working.

Main duties of the job

Clinical Care

Take patient histories, perform physical examinations, and make independent clinical diagnoses.Order diagnostic tests, interpret results, and prescribe medications as an independent prescriber.Refer patients directly to secondary care, specialists, or emergency hospital services.Holistic Enhanced Reviews

The PCN works together to deliver high-quality, patient-centred care to a diverse population, including a significant number of housebound patients, care home residents, and individuals with frailty and complex long-term conditions.

The ANP will deliver a dedicated service across the PCN, working autonomously within advanced clinical practice scope.

The role involves managing a caseload of patients, providing both planned and urgent care, and delivering holistic enhanced reviews, medication optimisation, and end-of-life care.

Main Duties and Responsibilities

Clinical duties

Conduct face-to-face, telephone, or video consultations for patients presenting with undifferentiated and undiagnosed problems Formulate clinical management plans, provide evidence-based care, and handle same-day urgent or acute presentations (such as minor illnesses, infections, and injuries) Prescribe safe and appropriate medications as an independent non-medical prescriber Order and interpret diagnostic tests (like blood tests or imaging) and make direct onward referrals to secondary care or specialist services Manage ongoing care for chronic illnesses such as diabetes, asthma, hypertension, and COPDHome visiting

Person Specification

Experience Essential

Primary care, community, or home visiting experience Management of frailty and complex long-term conditions Medication reviews and prescribing Palliative and end-of-life care

Qualifications Essential

Registered Nurse (NMC) MSc Advanced Clinical Practice (or equivalent) Independent Prescriber (V300)
