About this role

Job summary

Mersey care are looking for an enthusiastic, hardworking Band 4 Assistant Practitioner to join their innovative and dynamic Equipment Advisory Service (EAS), working to support Liverpool, Knowsley and Sefton Community Equipment Services. The positions could be suitable for an existing Assistant Practitioner or a Health Care Assistant with the right level of experience.

Your key role would be to support the qualified clinical equipment advisers in the assessment of adults and children for safe and suitable community equipment provision, providing and sharing your knowledge and skills in relation to assessment and provision of community equipment.

Ongoing training in relation to assessment skills and equipment available will be provided.

The service works collaboratively with many other services around Liverpool and are always open to new ways of working together to ensure safe and effective equipment is provided in a timely manner.

Main duties of the job

To give advice on appropriate community equipment within the patient home environment.

To support with the review of Care homes across all areas to ensure appropriate use of equipment and to encourage return of unused equipment.

To Support the prescribing of equipment in the community, to prevent hospital admissions and promote safe discharges from secondary care.

To assist the Clinical lead with training and education on safe and appropriate prescribing of equipment in the community setting.

To manage own caseload and work autonomously.

Provide clinical support and knowledge to support staff

Undertake risk assessments on defined patient in line with relevant guidelines.

Support with procurement exercises for all community equipment services.

Support with duty on a day to day basis responding to telephone and email enquiries.

Job description Job responsibilities

The post holder will have continuing responsibility for the management of a defined caseload under the supervision of a qualified practitioner. The primary function of the role is to support the responsive services of the Equipment Advisory Service. This will be done via a holistic assessment of care needs, joint assessments with qualified practitioners and supporting with the running of the EAS training programmes. The post holder will support qualified practitioners with community equipment reviews of the catalogue and ensuring equipment is issued and recorded effectively.

Practice autonomously and demonstrate evidence basedclinical decision making under the supervision of a qualified practitioner.

Work within guidelines to identify and manage risk, reporting identified risks to the team leader.

The post holder will demonstrate an attitude which respects and values service users' and their carers.

Person Specification

Qualifications Essential

Foundation Degree or equivalent or equivalent work based experience.

Experience Essential

Assessing, planning and implementing individualised programmes of care. Understanding local and national influences on health care delivery, including Public Health. Experience of previous work in health care Clinical Governance and Risk Management Actively involved in professional development

Desirable

Experience in assessing for and ordering community equipment.

Knowledge Essential

Knowledge of Health Promotion strategies Excellent communication, organisational and interpersonal skills Ability to travel, for work purposes

Desirable

Knowledge of developing self- care skills in patients/clients
