About this role

Job summary

We are looking to recruit a Band 6, Specialist Speech and Language Therapist (SLT), to a part time (0.6 whole time equivalent), permanent post within Learning Disability Speech and Language Therapy.

LDSLT are a small team of specialist SLTs, supported by dedicated Health Care Support Workers and Administrative colleagues. LDSLT sits in the Worcestershire Community Learning Disability Team and provides a county wide service. We are a small, friendly, and supportive team - who pride ourselves in being innovative, creative, flexible and person centred. You will hold a mixed caseload responding to both communication and dysphagia referrals. We enjoy good links with our SLT colleagues in neighbouring teams and have access to various networks providing clinical and professional support. We actively encourage our team to work proactively and respond creatively and flexibly to the changing demands of the role, delivering person centred episodes of care and working collaboratively with families, support workers and other health and care professionals.

Main duties of the job

You will provide assessment, diagnosis and develop bespoke therapy plans for those presenting with communication difficulties and/ or dysphagia arising from a range of conditions related to a learning disability.

Work with families, carers, and provider services in a variety of settings as needed to enable good outcomes. This may be creating communication passports, AAC both low and high tech and promoting Inclusive and Total Communication, Intensive Interaction, or dysphagia mealtime plan.

Maintain your own caseload within safe levels and support the team in prioritising referrals and managing the waiting list.

Contribute to social media and resource development, improvements in evidence-based practice and be proactive in personal and service delivery improvements.

Supervise and allocate work to assistants, students, and less experienced therapists.

Job description Job responsibilities

Assess, develop, and implement person centred, SLT treatment/ management using best interest and shared decision-making principles.

Make a differential diagnosis based on evidence from assessment, seeking advice as appropriate.

Use specialist knowledge to inform sound clinical judgements/ decision-making for case management.

Provide advice to others regarding the management and care of patients with communication disorder and/or with eating, drinking and swallowing difficulties (dysphagia).

Possess professional qualification in dysphagia management or a comparable level of competence (support to develop these will be available if needed).

Demonstrate effectiveness using evidence-based practice and outcome measures.

Communicate in suitable accessible format to patients, carers, and members of multi-professional teams.

Ensure that patients are involved in the planning and prioritisation of their care plans wherever possible.

Provide mentoring/coaching or training to support the implementation of care plans.

Write reports reflecting specialist knowledge.

Co-produce with service users, carers, staff, and others around individual case management.

Demonstrate ability to adapt practice to meet individual circumstances and implement reasonable adjustments as needed.

Demonstrate ability to reflect on practice and facilitate this ability in others.

Identify personal/professional development evidenced by Personal Development Plan developed within the appraisal framework, when required, including objectives relating to clinical specialisms.

Maintain up to date HCPC registration and RCSLT membership.
