Advanced Nurse Practitioner / Advanced Clinical Practitioner: Extended Access Hub, Belmont Health Centre, Harrow
Health Alliance PCN is looking for an experienced Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our Extended Access Hub, based at Belmont Health Centre in Harrow.
The service runs outside core GP hours and is mostly a minor illness clinic, with 15-minute appointments. You'll assess, diagnose, treat and manage a wide range of undifferentiated presentations autonomously, within your professional scope of practice, as part of the wider PCN team.
We welcome applications from suitably qualified ANPs and ACPs with the appropriate registration, advanced practice qualifications and primary care, urgent care or out-of-hours experience.
Service hours: Monday to Friday 16:00 to 20:00, Saturday 09:00 to 14:00
Working pattern: This is a regular, fixed-slot role. You'll be offered a set day or days each week (maximum of three shifts per week) and are expected to cover them consistently.
Main duties of the job
Assess, diagnose and treat patients aged 2 years and over presenting with minor illness and other undifferentiated, acute conditions, in 15-minute appointments. Review and manage patients with long-term conditions, as clinically appropriate within the Extended Access service. Work autonomously within your professional scope of practice, using sound clinical reasoning to manage risk and make safe decisions. Prescribe as an independent prescriber, in line with local formularies, national guidance and safe prescribing standards. Request and interpret investigations where clinically appropriate, and act on the results. Safety-net appropriately, and escalate or refer on to the right urgent, emergency or onward care when needed. Keep accurate, timely and contemporaneous clinical records on the practice clinical system (EMIS). Follow PCN and practice policies, including safeguarding, infection control, information governance and clinical governance. Work as part of the wider PCN team, communicating well with colleagues across the Extended Access Hub and member practices. Take part in audit, learning and service improvement where appropriate, and keep your own professional development and revalidation up to date.
This is a site-based clinic role at Belmont Health Centre; there are no home visits.

Keep accurate, timely and contemporaneous records on the clinical system (EMIS). Communicate clearly and compassionately with patients, carers and families, and with colleagues across the Hub and member practices. Work as a reliable member of the Extended Access team and attend your agreed sessions consistently. Pass on relevant information to the patient's registered practice where follow-up is needed. Follow PCN and practice policies, including safeguarding (adults and children), infection prevention and control, information governance, health and safety, and clinical governance. Report and learn from incidents, complaints and significant events. Take part in audit, quality improvement and service development where appropriate. Maintain your professional registration, revalidation, mandatory training and continuing professional development.
Person Specification
Experience Essential
Significant post-registration experience, including recent experience in primary care, urgent care, out-of-hours or an equivalent setting. Experience of independently assessing, diagnosing and managing undifferentiated patients, including minor illness. Experience of assessing and managing children aged 2 years and over. Experience of prescribing independently in a clinical setting. Experience of managing risk, recognising the acutely unwell patient, and escalating appropriately. Experience of working autonomously within your scope of practice. Experience of using an electronic clinical record system (EMIS, SystmOne or similar).
Desirable
Experience of working in an Extended Access, urgent care or walk-in setting. Experience of reviewing patients with long-term conditions in primary care. Experience of EMIS Web. Experience of audit or quality improvement work. Experience of working across multiple sites or with a range of teams.
