Job summary
We are seeking an enthusiastic independent prescribing pharmacist to join our expanding medicines management team at Ashbourne Medical Practice. Our current pharmacist is supported by a part-time clinical pharmacist, and we are expanding the team to meet the needs of our growing population.

Main duties of the job
We are a traditional GP practice dedicated to delivering safe, high-quality, patient-centred care. We prioritise easy access, effective signposting to additional services and technological innovation to improve efficiency. We use Welby Innovate recall, prescribing and pathology modules to automate workflow and to streamline safe medication monitoring, and reviews.
This role is practice based however there is some flexibility for hybrid working with a mix of on-site and remote work which is largely dictated by physical room space constraints.
About us
Based in the Derbyshire Dales, we are an established four-GP dispensing partnership providing general medical services under a GMS contract to people in Ashbourne and the surrounding areas. The practice operates from modern, purpose-built premises adjoining St Oswalds Hospital in Ashbourne. We are also a well-established teaching practice for GP registrars and medical students, supported by a strong clinical and non-clinical workforce.
Our patient list currently stands at 10,900 and continues to grow because of new housing developments within the practice boundary. The population profile is broadly in line with national statistics, although we have a slightly higher proportion of frail older patients. We serve both the town and surrounding rural communities, including a large farming population in nearby villages.

Clinical Responsibilities:
To work as part of a multi-disciplinary team in a patient-facing role taking responsibility for areas of chronic disease management within the practice.
To undertake structured medication reviews to proactively manage patients with complex polypharmacy, especially for older people and those with multiple co-morbidities.
To provide support to general practice staff with regards to prescription and medication queries.
Clinical pharmacist will have responsibility for overseeing AI and software algorithms generated searches (Welby Innovate) that automates workflow to streamline safe medication monitoring and review.
Clinical pharmacists will work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines
They will be prescribers, or will be completing training to become prescribers, and will work with and alongside the general practice team.
Clinical pharmacists will undertake medication reviews to proactively manage people with LTC, people in care homes, frailty, and people with learning disabilities or autism
Clinical Pharmacists will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practices and to help in tackling inequalities.
Clinical pharmacists will provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the QOF and both practice /PCN enhanced services.
Through structured medication reviews, clinical pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.
Clinical pharmacists will have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to
help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
The role has the potential to significantly improve quality of care and safety for patients.
The Clinical Pharmacist will develop relationships and work closely with other pharmacy professionals in the practice primary care network.
Clinical pharmacists will take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.
Take part in Clinical Audit and quality improvement to ensure prescribing is in line with local and national guidance, CQC requirements and up to date MHRA CAS alerts.
Support the wider clinical team with education and induction programmes for new clinical staff
To help support the repeat prescriptions system and deal with acute prescription requests.
To undertake medicines reconciliation on transfer of care and contribute to the development of systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).
To proactively work alongside the wider PCN, MSK and Mental Health multidisciplinary teams, on projects such as tackling health inequalities, low prevalence for CVD and high levels of opiates.
The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
