Main duties of the job

We are a total triage practice using GP Triage

Triaging Incoming requests via GP Triage

Signposting to appropriate healthcare professionals

15minute appointment sessions (15 appt per session plus three 10 minute appts for GP only to book)

6 weeks annual leave

Our nursing team plays a central role in the management of patients with chronic conditions, ensuring continuity of care and supporting improved health outcomes. We also provide a GP-led L2 Sexual Health Service to our patients. As an active member of a well established PCN we benefit from the support of a wide range of (ARRS) staff professionals

Location: Broomwood Health Centre

Job Purpose

The post holder will provide comprehensive general medical services to patients registered with the practice during the period of maternity leave cover.

The post holder will be responsible for the assessment, diagnosis, treatment and ongoing management of patients presenting with acute and chronic medical conditions, working within their professional competence and in accordance with GMC standards, NHS requirements and practice policies.

Key Responsibilities

Clinical Responsibilities

The post holder will:

Manage GP Triage dashboard / online consultations Provide consultations to patients presenting with acute, chronic and undifferentiated conditions. Undertake clinical assessment, diagnosis and management of patients. Provide face-to-face, telephone and other appropriate forms of consultation. Prescribe safely and appropriately in accordance with current clinical guidance. Refer patients appropriately to secondary care, community services and other healthcare professionals. Arrange appropriate investigations and interpret results within the scope of professional competence. Provide appropriate follow-up and safety-netting advice. Undertake management and review of patients with long-term conditions as required. Manage urgent and same-day clinical presentations according to practice arrangements. Undertake home visits where required and agreed within the practice's arrangements. Maintain accurate, contemporaneous and clinically appropriate patient records. Complete clinical correspondence, referrals, reports and other documentation arising from consultations. Review and action investigation results, correspondence and other clinical information within agreed practice systems. Recognise and appropriately manage patients requiring urgent or emergency care. Clinical Governance and Quality

The post holder will:

Participate in clinical governance and quality improvement activities. Contribute to significant event reviews and clinical audits where appropriate. Work in accordance with local and national clinical guidelines. Comply with practice policies and procedures. Participate in safeguarding processes and raise concerns appropriately. Maintain appropriate standards of infection prevention and control. Report clinical incidents, risks and significant events in accordance with practice procedures. Maintain professional competence and undertake appropriate continuing professional development. Participate in appraisal and revalidation in accordance with GMC requirements. Team Working and Communication

The post holder will:

Work effectively as a member of the practice's multidisciplinary team. Communicate appropriately with GP colleagues, nursing staff, pharmacists, allied health professionals, administrative staff and external healthcare providers. Participate in practice and clinical meetings as required. Contribute to effective communication and continuity of patient care. Provide appropriate clinical advice and support to other members of the clinical team within the scope of professional competence. Professional Responsibilities

The post holder will:

Maintain full compliance with GMC professional standards. Maintain patient confidentiality at all times. Comply with information governance and data protection requirements. Work within the limits of professional competence and seek appropriate advice when required. Maintain appropriate professional indemnity arrangements. Comply with practice health and safety policies. Comply with safeguarding policies and procedures. Undertake duties in accordance with the practice's equality, diversity and inclusion policies. Scope of the Role

The post holder will undertake the duties normally expected of a GP working within the practice, subject to the agreed sessional commitment and individual competence.

The exact clinical workload, consultation model, home visiting arrangements, duty doctor responsibilities and administrative workload will be determined by the practice's operational arrangements and the terms of the fixed-term contract.

Qualifications and Professional Registration

Essential

GMC registration with a licence to practise. Inclusion on the NHS England Medical Performers List. MRCGP or equivalent. Appropriate professional indemnity arrangements. Satisfactory completion of relevant employment and pre-employment checks. Experience

Experience of working in UK general practice. Experience in the assessment and management of a broad range of acute and chronic conditions. Experience of multidisciplinary working. Experience of working within clinical governance and safeguarding frameworks. Knowledge and Skills

Good knowledge of current general practice. Ability to assess and manage undifferentiated clinical presentations. Knowledge of appropriate prescribing and referral pathways. Understanding of clinical governance, safeguarding and patient confidentiality. Effective clinical communication skills. Ability to work independently and as part of a multidisciplinary team. Ability to prioritise workload and manage clinical risk appropriately. Accountability

The post holder is professionally accountable to the GMC and clinically accountable within the practice structure to the GP Partners.

The post holder is expected to work within their professional competence and to seek appropriate clinical advice where necessary.
