An exciting opportunity has arisen for a Nursing Associate to join the West Norfolk Community Mental Health Team (CMHT).
Working as part of a multidisciplinary team, you will support adults experiencing mental health difficulties by delivering high-quality, person-centred and recovery-focused care within community settings. You will contribute to assessment, care planning, physical health monitoring, risk management and therapeutic interventions, helping service users to achieve their recovery goals and maintain their wellbeing.

Main duties of the job
As a Nursing Associate within the Community Mental Health Team (CMHT), you will play a key role in delivering high-quality, recovery-focused care to adults experiencing a range of mental health difficulties within the community. Working as part of a supportive multidisciplinary team, you will help service users to achieve their goals, promote independence, and support positive mental and physical health outcomes.
Key responsibilities include:
Supporting the assessment, planning, delivery and review of care for individuals with mental health needs. Building therapeutic relationships with service users, carers and families to promote engagement and recovery. Monitoring service users' mental and physical health, recognising signs of deterioration and escalating concerns appropriately. Supporting the completion of physical health monitoring and promoting healthy lifestyles. Contributing to risk assessment and risk management plans, ensuring concerns are appropriately identified and acted upon. This is an exciting opportunity for a motivated Nursing Associate who is passionate about supporting people with mental health difficulties and who enjoys working autonomously within a dynamic and collaborative community-based service.
