About this role

Job summary

To assist the senior team in providing care and physical rehabilitation to people who require short team period of recovery in their own homes following an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live asindependently as possible.

Main duties of the job

To work as part of the multidisciplinary team and when necessary on own initiative.To provide direct care and rehabilitation to the patients served by the team.To act in a manner that protects and maintains the patient's sense of dignity and respect, while observing their individual religious and cultural practices.

Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.Ensure active involvement in manual handling of patients as required - in line with the Trust's Manual Handling Policy.To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.Observe record and report changes in the patients' health and environment to senior staff.To be familiar with the steps to be taken in medical emergencies and act accordingly.Complete observations including urinalysis and blood sugar monitoring as required.

Assist the senior team with prelim visits, first visits and review visits to patients.Work directly with the therapists within the team to support them providing effective assessment and treatment including in the in-patient beds if necessary.Assist in planning work together with the senior team.Assist in contributing to care planning and risk management plans.

In accordance with EPUT confidentiality policy. Being aware that any information received whilst on duty if confidential.Act as direct support to senior members of the team including the Team Leader, associate practitioners, nurses and therapists to ensure the effective service provision.RESPONSIBILITY FOR PATIENT/CLIENT CAREEncourage patients to participate in activities of daily living and facilitate their activeinvolvement in therapeutic activities.Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.Ensure active involvement in manual handling of patients as required in line with the Trusts Manual Handling Policy.To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.Observe record and report changes in the patients health and environment to senior staff.To be familiar with the steps to be taken in medical emergencies and act accordingly.Complete observations including urinalysis and blood sugar monitoring as required.

Collect patient specimens as required.Observe and record dietary needs and fluid intake as required.Assist the senior team with prelim visits, first visits and review visits to patients.Work directly with the therapists within the team to support them providing effectiveassessment and treatment including in the in-patient beds if necessary.Assist in planning work together with the senior team.Assist in contributing to care planning and risk management plans.

COMMUNICATION AND WORKING RELATIONSHIPSEnsure excellent communication skills with patients and carers at all times.Ensure adherence to record keeping policy and the use of Systm One achieving accurate records and reporting any changes to senior staff with due regard to confidentiality.Establish and maintain a good professional working relationship with colleagues and other professionals both internal and external to the team.Be responsible for working with colleagues, seeking to improve the quality of services provided to our patients.Be responsible for communicating accurate information assisting with office based tasks including the team data base, archiving etc.Be aware of your working environment and report any hazardous situations to the senior in charge.

Be flexible co-operative and willing in undertaking allocated team tasks.Assist with induction and teaching of new staff/visitors.

INFORMATION SYSTEMSTo be able to use a range of IT systems as required by the service to support effective working.To be able to communicate electronically, i.e. using emails and use of trust intranet.To be able to complete all required electronic records on Systm One.

Person Specification

Knowledge/education/qualifications Essential

Good standard of education to GCSE or equivalent

Desirable

Completed Care Certificate NVQ3 in Health and Social Care

Skills/experience Essential

Experience providing care in a Community Setting Experience in rehabilitation with a positive approach to rehabilitative care

Desirable

Experience with working with Stroke patients Previous experience of System one

Personal Qualities Essential

Holds current UK driving licence Access to vehicle to use for work purposes
