An exciting opportunity has arisen for a highly motivated, enthusiastic and passionate clinical pharmacist with a keen interest in renal services to join our expanding integrated pharmacy team. You will become part of a unique team in a highly patient focused role. You will be responsible for leading and developing clinical pharmacy services to patients with renal disease across the Central Integrated Health Community (IHC).

The Central IHC manages patients with chronic kidney disease, including those who require advanced treatment options, such as dialysis. The successful candidate will provide support to the multidisciplinary team for these patients whilst inputting into transformative changes in how care is being delivered across the IHC.

The successful candidate will be a specialist clinical pharmacist, with or able to work towards an independent prescribing qualification. You will be able to work well as part of a ward-based team and across the wider Central IHC, providing leadership and clinical expertise to other pharmacists and technicians within the IHC

Main duties of the job

The successful candidate will be supported to develop the scope of the role, supporting the renal unit, Emergency Quarter and wards to review patients with acute and chronic kidney disease who are admitted to hospital and in community settings.The renal unit at Glan Clwyd treats patients with varying degrees of renal function and those requiring acute or chronic dialysis treatments. You will have a key role within the multidisciplinary team of doctors, specialist nurses, dieticians and other allied healthcare professionals.You will lead and manage high cost medications within the renal pharmacy service and provide evaluated financial information in accordance with the needs of the division.You will be self-motivated with excellent communication, time management and interpersonal skills. The ability to liaise with and influence senior clinical staff, members of the multidisciplinary team and colleagues with various clinical background across all sectors of the Health Board - including and not limited to primary, community and secondary care is essential.The successful candidate will benefit from working in a transformational department and our aim is that all of our pharmacy team maximise their potential.The pharmacy team are closely integrated into the multidisciplinary team and there are opportunities to support with guideline, protocol and service developments across Betsi Cadwaladr University Health Board.

Qualifications Essential

MPharm degree or equivalent Registered with the General Pharmaceutical Council Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications

Desirable

Member of the Royal Pharmaceutical Society & registration with the Faculty Non-medical prescribing qualification (or working towards)

Experience Essential

Must be able to demonstrate suitable level of experience working in the managed sector and/or community pharmacy

Desirable

Experience in relevant clinical area Leadership and management experience

Aptitude and Abilities Essential

Good verbal/written communication skills IT literate Evidence of implementation of change Ability to prioritise and delegate Development of protocols and procedures Ability to set and meet deadlines

Other Essential

Participate in the provision of general public holiday and weekend pharmacy services as necessary. Willingness to participate in "on call" out of hours duty roster as necessary
