Job summary

Are you an experienced and patient-focused Clinical Pharmacist looking to develop your career within a progressive and supportive primary care team?

The Old School Surgery is seeking a motivated Clinical Pharmacist to join our highly skilled multidisciplinary team. You will join a large, valued and supportive clinical pharmacy team (6 Pharmacists and 2 Pharmacy Technicians) based solely at Old School Surgery and play a key role in medicines optimisation, patient safety, long-term condition management and prescribing quality improvement, working collaboratively with GPs, nurses, care coordinators and other allied health professionals to improve outcomes for our population of over 32,000 patients.

This is ideally a full-time position, across 4 or 5 days, but consideration will be given to excellent candidates who may prefer slightly fewer hours.

Main duties of the job

As a Clinical Pharmacist, you will provide expert pharmaceutical care and demonstrate safe clinical decision-making within general practice. Working within your professional competence and scope of practice, you will:

Undertake structured clinical medication reviews for patients with complex and long-term conditions.Provide medicines optimisation support to improve patient outcomes and reduce medicines-related harm.Manage repeat prescribing processes and support safe prescribing systems.Support the management of long-term conditions where medicines optimisation is required.Implement medicines safety alerts, national guidance, NICE recommendations and local formulary changes.Contribute to quality improvement initiatives and clinical audits.Provide education and medicines advice to clinicians, practice staff and patients.Contribute to public health initiatives, including vaccination campaigns and health promotion activities.Why Join Us?

Work collaboratively with community pharmacies, secondary care colleagues and wider healthcare teams to ensure continuity of patient care.

Person Specification

Experience Essential

Minimum of two years post-registration pharmacy experience. Excellent clinical knowledge and understanding of medicines optimisation. Experience of patient consultation and medication review. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Good IT and clinical system skills. Ability to analyse clinical information and make evidence-based recommendations.

Desirable

Previous experience working within primary care or general practice.

Qualifications Essential

MPharm degree or equivalent pharmacy qualification. Current registration with the General Pharmaceutical Council (GPhC).

Desirable

Hold or be willing to work towards Independent Prescriber qualification. Clinical Diploma or postgraduate qualification. Have completed or be willing to complete the CPPE PCPEP pathway.
