Job summary
We are looking for an experienced, motivated and caring health care assistant to join our friendly Neighbourhood Teams.
These posts are offered on a six-month fixed-term basis. There may be an occasional requirement to work at other locations across the county to support service delivery, depending on operational capacity.

Your role within the team is essential in allowing patients to achieve optimum function within their own home.
The Neighbourhood Team operates from 07:30-18:30 each day and you will work 07.30-15.30, 08:30-16:30 or 10:30-18:30 shift patterns on a roster basis.
Our Neighbourhood team consists of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistant's and administrative staff; with GP and social care colleagues working closely alongside.

Main duties of the job
As a Band 3 Healthcare Support Worker, you will play an important role in delivering high-quality, person-centred care to patients in their own homes, helping them to maintain their independence, dignity, safety and wellbeing.
Working both independently and alongside colleagues, under the direction of registered nurses and therapists, you will complete delegated care and clinical tasks in accordance with individual care plans, Trust values, policies and procedures. Following appropriate training and competency assessment, your duties may include supporting personal care and activities of daily living; taking, monitoring and recording clinical observations; providing wound and catheter care and simple dressings; undertaking venepuncture; administering medication; supporting safe moving and handling; and fitting and demonstrating the safe use of equipment.
You will develop positive working relationships with patients, carers and families, recognise and promptly report any changes or concerns relating to a patient's condition, contribute to care planning and reviews, and communicate effectively with the wider multidisciplinary team. You will also maintain accurate, timely and contemporaneous records using EMIS Community, while promoting patient choice, confidentiality, infection prevention and consistently safe, compassionate care.

Person Specification
Qualifications Essential
Level 3 qualification (e.g. NVQ 3, A-level) in Care or equivalent qualification or experience Or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust

Experience Essential
Experience of working with older adults in a health/social care setting

Community experience of older adults with physical and/or mental health problems Experience of working and liaising with staff from different disciplines
