BRUNEL PRIMARY CARE NETWORK is seeking an organised, proactive and compassionate Primary Care Coordinator to join our Integrated Neighbourhood Team, based at Milton Park Surgery.
This is an exciting opportunity to become part of a multidisciplinary team delivering coordinated, proactive care to some of the most vulnerable members of our community, including frail and housebound patients, those receiving palliative care and individuals experiencing homelessness.
Main duties of the job
The successful candidate will provide valuable organisational, administrative and care coordination support to ensure patients receive timely, joined-up care and that clinicians are supported to deliver effective services.
The role will involve maintaining and managing registers of patients at risk, coordinating appointments, particularly for our Same Day Acute Visiting Service, contacting patients to arrange visits, supporting the organisation of Long-Term Condition (LTC) management and arranging Gold Standards Framework (GSF) meetings.
You will also support clinical data collection, patient monitoring and accurate record keeping and coding on SystmOne.
We are looking for someone with excellent organisational and communication skills, who is confident managing competing priorities, has a compassionate approach to patient contact and enjoys working as part of a wider multidisciplinary team.
Previous experience working in primary care and knowledge of SystmOne would be advantageous but are not essential, as training and support will be provided for the right candidate.

The Brunel Primary Care Network consists of three Practices across Portsmouth; East Shore Partnership, The Lighthouse Group and University Surgery.
We have highly skilled and motivated teams working within our PCN, which consist of GP Partners, Salaried GP's, Advanced Nurse Practitioners, Pharmacists, Diabetes and Respiratory Nurse Specialists, Nurses, GP Assistant's and HCA's; who are supported by skilled administration teams.

Key Responsibilities
Patient Coordination and Proactive Care
Maintain and manage registers of patients identified as being at increased risk, ensuring information is accurate and up to date. Support the Integrated Neighbourhood Team in identifying patients requiring proactive follow-up and coordinating their care. Provide administrative and coordination support to ensure effective delivery of proactive and responsive care. Liaise with patients, carers, practice teams and wider healthcare professionals to support continuity of care. Support the coordination of care for frail, housebound, palliative and clinically vulnerable patients.Appointment and Service Coordination
Support appointment management, particularly for the Same Day Acute Visiting Service, ensuring appointments are coordinated efficiently. Contact patients and carers by telephone to arrange appointments, home visits and follow-up care. Assist with the organisation and coordination of clinical activity to ensure effective use of available appointments and resources. Support communication between clinicians, practices and patients to facilitate timely care.Long-Term Condition and GSF Management
Support the organisation and coordination of Long-Term Condition (LTC) management, including assisting with patient lists, recalls, appointments and monitoring activity. Coordinate and arrange Gold Standards Framework (GSF) meetings, including scheduling meetings, preparing patient lists and ensuring relevant information is available to the clinical team. Support the recording and follow-up of actions arising from GSF and multidisciplinary team meetings. Assist with maintaining accurate patient registers to support proactive care planning and clinical reviews.Data, SystmOne and Administration
Assist with the collection, extraction and collation of clinical and service data to support reporting, monitoring and service development. Support accurate clinical coding and record keeping on SystmOne, ensuring patient care and interventions are appropriately documented. Assist with monitoring activity and maintaining accurate records to support service delivery and reporting requirements. Maintain confidentiality and ensure all patient information is handled in accordance with information governance requirements. Provide general administrative and organisational support to the Integrated Neighbourhood Team as required.

Experience Essential
Previous experience in an administrative or coordination role.
Desirable

Qualifications Essential
Educated to GCSE grade A to C in English and Maths or equivalent.
