MPFT INTERNAL STAFF ONLY
An exciting opportunity has arisen for an enthusiastic and motivated NMC Registered Band 6 Nurse to join our highly respected Community Diabetes Specialist Service across Staffordshire & Stoke on Trent.
This is an excellent opportunity to become part of a multidisciplinary team comprising Specialist Nurses, Consultants, Diabetes Educators, and Administrative colleagues, all working together to deliver high-quality, patient-centred diabetes care across our communities.
Our service provides expert diabetes assessment, management, education, and support through nurse-led clinics, home visits, and collaborative working with other teams. We are committed to improving outcomes for people living with diabetes through evidence-based practice, innovation, and education.
We are looking for a nurse with demonstrable experience in diabetes care and management, who is passionate about supporting patients and developing the knowledge and confidence of healthcare professionals. An accredited diabetes qualification would be advantageous; however, we would also welcome applications from nurses with significant diabetes experience and a strong commitment to further development within this specialist field.
This role offers excellent opportunities for professional growth, including the development of advanced clinical skills and specialist diabetes knowledge. A Non-Medical Prescribing qualification would be beneficial but is not essential.
Main duties of the job
As a Band 6 Diabetes Specialist Nurse, you will:
Deliver specialist assessment, treatment, and ongoing management for people living with diabetes. Assess complex health needs and develop, implement, and evaluate individualised care plans. Provide expert advice and support to patients, carers, community services, and primary care colleagues. Promote self-management and empower patients to achieve positive health outcomes. Contribute to the education, training, and mentorship of healthcare professionals and patients. Support service improvement initiatives and contribute to the continued development of diabetes services. Demonstrate clinical leadership, professional accountability, and evidence-based practice. Work collaboratively with multidisciplinary colleagues to achieve service objectives. Use initiative, creativity, and problem-solving skills to support high-quality patient care. As this is a community-based role, you will be required to travel independently across a large geographical area.
