Job summary
We are recruiting ACPs on a fixed term contract until 31st August 2027 to join our established and developing Primary Care service within the Rotherham Urgent and Emergency Care Centre (UECC). This is an exciting opportunity to work as part of a well supported MDT.
ACPs within our service work across an extended-hours rota, including evenings and weekends, with shifts running up to midnight.
This is a varied and rewarding clinical role. ACPs contribute across key areas of the service including Urgent Primary Care, GP Out of Hours and front-door streaming within the Emergency Department/UECC.
The role requires confident clinical assessment, decision-making and risk management, alongside appropriate escalation to GP or Emergency Department colleagues where required. You will work as an autonomous practitioner making appropriate referrals, initiating treatment and facilitating safe discharge, supported by critical thinking and an evidence-based approach.
We are looking for ACPs who are confident working with undifferentiated patients, demonstrate sound clinical judgement and are able to work effectively within a MDT. Applicants should hold an MSc in Advanced Clinical Practice, or equivalent and registered with either the NMC or HCPC.

Main duties of the job
This role involves providing high-quality clinical assessment, intervention and treatment planning for undifferentiated patients, including children presenting to the primary care stream of the UECC and/or the GP Out of Hours service. You will work autonomously, making appropriate referrals or discharging patients safely while using advanced clinical judgement and an evidence-based approach.
You will be expected to work across services, delivering a wide range of consultation skills including:
Front-door streaming.
Face-to-face consultations.
Telephone triage and consultations, including patients referred via NHS 111.
Booked clinic appointments.
Home visiting within the local area.
You will also be expected to support the development of trainee ACPs and junior colleagues, and to contribute to safe, effective service delivery across the wider team.
