Job summary
As a Team Coordinator, you will provide comprehensive administrative and operational support across Intermediate Care, Discharge and Integrated Care pathways. Reporting to the Service Team Manager, you will play a key role in supporting system flow, coordinating referrals, maintaining performance and pathway tracking systems, and ensuring effective communication between health and social care partners.
Working within a fast-paced environment, you will help ensure residents receive timely and coordinated support whilst maintaining accurate records and supporting service performance.
Main duties of the job
Receiving, processing and coordinating referrals across Adult Social Care pathways. Maintaining service trackers, waiting lists and performance monitoring systems, escalating risks and delays where required. Updating and maintaining records on Mosaic and other electronic systems to ensure accurate and timely information. Coordinating communication between hospital teams, community services, care providers and Adult Social Care colleagues. Providing administrative support to operational and multidisciplinary meetings, including preparing reports, agendas and documentation.

This is an excellent opportunity for two enthusiastic Team Coordinators to join our Adult Social Care Hospital Team, supporting Intermediate Care, Discharge and Integrated Care services.

Referral and Discharge Coordination
Receive, process and with guidance allocate referrals in accordance with agreed procedures, MDT decisions and timescales.
Monitor referral activity and escalate delays, risks or capacity concerns appropriately.
Support the coordination of team pathways.
Ensure accurate recording and tracking of progress through pathways.
Assist operational teams in achieving timely and safe outcomes.
Tracker Management and System Flow
Maintain and update team trackers and information systems on a daily basis.
Monitor resident status and pathway progression.
Support accurate activity reports and performance information.
Support system-wide visibility of demand, capacity and delays.
Identify data anomalies and ensure corrective action is taken promptly.
Mosaic and Case Management Administration
Create, update and maintain records on Mosaic and other relevant case management systems.
Ensure statutory recording requirements are completed accurately and within required timescales.
Maintain data quality and compliance with organisational policies.
Communication and Customer Contact
Manage dedicated service telephone lines and shared inboxes.
Respond professionally and appropriately to enquiries from residents, families, carers and professionals.
Facilitate communication between hospital teams, community services and care providers.
Escalate urgent concerns to operational leads where required.
Performance and Reporting
Produce routine and ad hoc performance reports.
Support reporting requirements relating to team activity.
Assist managers with data collection, monitoring and service improvement activity.
Prepare documentation and information for meetings and operational reviews.
Service Support
Provide administrative support to operational meetings and multidisciplinary discussions.
Support service development initiatives and implementation of new processes.
Contribute to continuous improvement activities that enhance flow and service efficiency.
Provide cross-cover across other operational teams as required.
Other Responsibilities
Maintain confidentiality and comply with data protection legislation.

Participate in supervision, appraisal and mandatory training.

Person Specification
Qualifications Essential
GCSE English and Mathematics (or equivalent). (Essential) NVQ Level 3 in Business Administration or equivalent experience. (Desirable) Proficient in use of MS Teams and Microsoft office applications. (Essential)
Experience Essential
Experience working in a fast-paced environment. Working with multiple people in various roles. Experience of supporting a multi-disciplinary team. Experience of maintaining databases, trackers and electronic records. Experience of handling sensitive and confidential information. Experience of communicating with professionals and members of the public.
Desirable
Significant administrative experience within health, social care or related services. (Desirable) Experience of managing referrals, caseload information or workflow systems. (Desirable)
