About this role

Job summary

As a Medical Notes Summariser, you'll play an important role in ensuring our patient records are accurate, clear and up to date, helping our clinical teams quickly access the information they need to provide safe and effective patient care.

You'll review and summarise medical records, maintain accurate problem lists, record relevant clinical information and help improve the overall quality of our patient records.

As the practice operates across Keresley Green, Keresley End and Fillongley, you will need to be able to work across our sites. You'll be part of a friendly, professional team where accuracy, confidentiality and attention to detail are essential, and where the work you do behind the scenes makes a genuine difference to both patients and clinicians.

The role also offers variety beyond day-to-day summarising, including supporting the maintenance and improvement of patient records, adding immunisation and seasonal vaccination information, undertaking audits and helping us continually improve the quality of the information held within our clinical system.

Main duties of the job

The Medical Notes Summariser will be responsible for reviewing patient medical records and ensuring key clinical information is accurately captured and easy for clinicians to access. The role will involve summarising records, maintaining accurate and relevant problem lists and helping to ensure patient information is clear, consistent and up to date.

You will also support the quality of our clinical records through audit work, immunisation and vaccination data entry, and identifying information that may require clarification or escalation to a clinician. You will manage records through the summarising process, ensuring they are appropriately tracked, filed, retained or transferred as required.

This is a role requiring excellent attention to detail, confidentiality and the ability to work independently while recognising when clinical input is needed.

Job description Job responsibilities Role Summary

To accurately review, summarise and input clinical information from patient medical records, ensuring that notes are clear, concise and accessible to support effective patient care and clinical decision-making.

Key ResponsibilitiesReview and summarise patient medical notes, including clinical letters, test results and hospital discharge summaries.Review and clean up patient medical records by ensuring that the problem list is accurate and up to date, removing duplicate or irrelevant entries and streamlining notes to improve clarity and usability for clinicians.Ensure that only relevant and active clinical problems appear on the problem list, supporting effective patient care and decision-making.Ensure summaries are accurate, clear and compliant with clinical and data governance standards.Assist with backlog clearance and ongoing summarisation tasks to support clinicians and improve record quality.Add bulk childhood immunisations and seasonal vaccines onto the clinical system.Undertake audits of work to ensure compliance and support quality improvement projects.Identify and promptly flag any discrepancies, safeguarding concerns or urgent clinical issues to the appropriate member of staff.Manage patient medical records received for summarising, ensuring they are processed and tracked appropriately and, once completed, filed or retained within the practice or returned/transferred to the appropriate organisation as required.General AdministrationAttend meetings as required.Attend relevant training sessions.Keep up to date with all statutory and mandatory training.Maintain confidentiality at all times.Work in line with all practice policies and standard operating procedures.Complete any other reasonable management instruction.

Person Specification

Experience Essential

Experience of working with clinical records or medical documentation, ideally within a healthcare setting. Good understanding of medical terminology, confidentiality and the handling of sensitive patient information.

Desirable

Previous experience of summarising medical records within general practice. Experience of clinical coding and electronic patient record systems, preferably EMIS Web. Previous experience of working within general practice or primary care.

Qualifications Essential

Good standard of general education, including English and Maths (or equivalent). Training in medical terminology, or demonstrable equivalent knowledge gained through relevant experience.

Desirable

Relevant qualification in medical terminology, medical/healthcare administration or health records management. Information Governance/Data Protection training.

Knowledge & Skills Essential

Good understanding of medical terminology. Excellent attention to detail and accuracy when reviewing and recording clinical information. Ability to identify and accurately record relevant information from clinical correspondence and patient records. Good IT skills and ability to use electronic patient record systems. Ability to recognise when information requires clarification or escalation to a clinician.

Desirable

Knowledge of GDPR, DSPT and NHS Information Governance requirements.
