Position Summary As a Licensed Practical Nurse (LPN) or Graduate Practical Nurse (GPN) on our Chronic Care team, you'll provide compassionate, patient-centered care to individuals with complex, long-term medical needs. Working under the supervision of a Registered Nurse, you'll collaborate with the interdisciplinary team to assess patient needs, implement individualized care plans, monitor patient progress, and support positive clinical outcomes. At HFSC, you'll build lasting relationships with patients while gaining opportunities to expand your skills and explore career growth across our specialty programs.

Qualifications:

• Required: Graduate of an accredited licensed practical nurse program • Required: Current licensure as a licensed practical nurse in the State of Connecticut • Preferred: Valid/Current CPR certification -required within three months of employment • Preferred: Minimum one year LPN experience in a medical/surgical or long-term care setting.

COMPENSATION & BENEFITS

At Hospital for Special Care, we are committed to supporting the health, financial well-being, and professional growth of our team members. The wage range for this position is $26.75 to $40.12.
