About this role

Job summary

The post holder will be an integral part of the Urology Cancer Admin Team, who assists, supports and co-ordinates the pathway for prostate cancer patients who have completed primary treatment. A large part of the role is to assist patient to transfer onto a supported self management programme to reduce the number of visits to hospital . They will be providing administrative and co-ordinated support to patients, families, consultants, cancer nurse specialists and other staff involved in the prostate cancer pathway.

They will co-ordinate/act independently to make OPD appointments for those with abnormal results, fast track patients back into the system if required,.

Ensure system is kept updated with all patient interactions and that reminder letters for repeat tests are sent out in timely manner.

Administer fail safe to ensure patients' results are registered accurately and action taken appropriately.

Coordinate patient Health MOT information and respond to any areas of concern that have been highlighted by patient.

Registration of patients onto relevant databases ensuring all patient data both clinical and personal is electronically and accurately recorded

Main duties of the job

Be the point of contact for all enquires relating to the supported self management system for prostate patients. Develop the knowledge and skills on how patients can be transferred onto this system ensuring that they fully understand how it works and the benefits to them.

Facilitating communication and information flows, initiating and responding to correspondence/emails and resolving enquiries.

Develop and maintain effective working relationships with clinicians, managers and others within department and Trust wide service users.

Maintain an accurate database/portal of patients including systems to track interval investigations and monitor incoming health status information from patients using the system.

Specifically assist the ANP's in co-ordinating care by tracking patient pathways and providing a point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.

Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways

Provide basic telephone advice and refer on or sign-post to other sources of support

Have excellent I T , counselling and persuasive skills

Person Specification

Qualifications Essential

Diploma level qualification or equivalent in relevant field/discipline GCSE English and Mathematics or equivalent ECDL or equivalent experience

Desirable

Evidence of continued role development Willing to undertake Macmillan learning and development courses. Basic counselling NVQ in Care or similar

Experience Essential

Relevant Health or Social Care experience Experience of coaching/teaching patients and carers

Desirable

Previous medical secretarial experience Understanding of Macmillan Cancer Support and its role across the UK. Experience co-ordinating a patient workload. Awareness of Improving Outcomes; A Strategy for Cancer and other national cancer policies Knowledge of relevant cancer treatments, interventions and terminology.

Knowledge Essential

Efficient in the use of Microsoft Word for Windows, Excel, Power Point, Outlook and internet. Advanced knowledge of medical terminology. Understanding of person-centred care.

Skills Essential

Able to use own initiative and work without supervision. Able to prioritise work and work of others. Possess excellent communication skills. Effective organisation skills. Ability to communicate both verbally and non-verbally on a daily basis with people at all levels. Good observation skills.
