As our services continue to grow in response to community need, we are looking to appoint a qualified and experienced Registered Nurse to join our Nursing Care Services as a Registered Nurse Care Coordinator.

Main duties of the job
Working as part of the Nursing Service at Tynedale Hospice at Home, the Registered Nurse Care Coordinator plays a key role within the care coordination team, supporting the delivery of high-quality, person-centred care.
The role involves triaging referrals, coordinating care and managing a caseload, ensuring patients receive timely and appropriate support. This includes allocating care to Registered Nurses, Healthcare Support Workers and an Apprentice Senior Healthcare Support Worker, based on individual patient needs.
This is a varied role combining office-based coordination with clinical practice, including patient assessment and review, and effective use of digital systems to support safe and responsive care delivery. The post holder will also contribute to the development and support of junior staff, including mentoring responsibilities, and participate in the THH Traceability rota.
Working closely with multidisciplinary teams across West Northumberland, the post holder will support integrated working, helping to improve access to high-quality palliative and end-of-life care, ensuring patients and families receive compassionate, coordinated support when they need it most.
You must hold a full driving licence and have access to a car insured for business use.

General Statement:
Tynedale Hospice at Home (THH) Registered Nurses (RN) are integral members of the multi-disciplinary team providing supportive and palliative care to a range of patients with life limiting illnesses.
Working within the THH Nursing Service, this role forms part of the RN Care Coordinator team, coordinating care through triage, ongoing patient management, and allocation of care to RNs, Hospice Support Workers (HSW) and Level 3 Apprentice Senior Healthcare Support Worker.
The post incorporates both office-based and clinical responsibilities, including care coordination, patient assessment and review, utilisation of IT systems to support care delivery, mentorship of junior staff (including an Apprentice Senior Healthcare Support Worker), peer supervision / support when required and participation in the THH Traceability rota.
Main Purpose of the role:
The post holder will work collaboratively with multidisciplinary teams across West Northumberland to enhance service integration and improve timely access to high-quality, patient-centred palliative and end-of-life care. This includes supporting the development of integrated services that promote earlier referral to community and hospice care, helping to reduce inappropriate hospital admissions and prolonged stays in acute settings.
Key Responsibilities:
Care Coordination and Clinical Practice
Triage inbound referrals and liaise with patients, families and carers to coordinate and deliver high-quality nursing care. Assess, plan, implement and evaluate care to meet individual patient needs within scope of practice. Undertake patient visits to complete assessments and reviews, working collaboratively with community teams and wider THH Care team to ensure continuity of care. Monitor and evaluate end-of-life care plans to ensure effectiveness of interventions. Delegate care appropriately to RNs and HSWs. Work autonomously, using clinical judgement while accessing support when required. Maintaining concise and accurate records of care delivery within electronic patient record, including Caring for the Dying Patient documentation Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability. Participate in the THH Traceability rota (approximately every 4-5 weeks).Communication & Patient Support
Communicate sensitively with patients, families and carers, recognising the impact of death, dying and bereavement. Manage complex, sensitive or unpredictable situations effectively. Support communication needs, including those with language or communication barriers. Facilitate discussions relating to complex and emotive aspects of end-of-life care.
Assessment and Care Planning
Understand the range of assessment tools and ways of gathering information, and their advantages and disadvantages Assess pain and symptom management using appropriate tools and clinical judgement. Understand and apply a range of assessment methods to inform care delivery. Undertake/contribute to multi-disciplinary holistic assessment and information sharing
Advance Care Planning
Demonstrate knowledge of Advance Care Planning, including legal and ethical considerations under the Mental Capacity Act. Support informed decision-making through clear and appropriate communication. Work sensitively with patients and families to support preferences and wishes.
Leadership, Supervision and Service Development
Support the Deputy Head of Care in maintaining care standards and service quality. Supervise, support and appraise RNs, HSWs and Apprentice staff. Provide mentorship and promote evidence-based practice through professional development and knowledge sharing. Contribute to service development, audit and quality improvement initiatives. Support the collection and reporting of key performance indicators. Participating in policy, protocol and procedure development where required. Promotes health and safety, undertaking risk assessments as required.Professional Development
To undertake mandatory training as required To undertake appropriate personal development to maintain and develop skills and knowledge as determined by annual appraisal. This is subject to the availability of resources and requirements of the Department.

Person Specification
Qualifications Essential
First Level Registered Nurse with minimum of 3 years post registration experience. Evidence of continuous personal and academic development.
Desirable
Palliative Care qualification.
Knowledge and skills Essential
Ability to interface effectively with a range of people in complex situations. Excellent IT skills including working knowledge of Microsoft Office applications.
Desirable
Knowledge of healthcare IT System
Experience Essential
Experience of working without direct supervision in a health care environment. Awareness of Palliative Care and End of Life Care Issues.
Desirable
Experience of working in the community. Experience of teaching or mentorship in clinical practice.
