What you’ll do?
Every day as a Senior Care Assistant is different, but your impact is always the same - life-changing.
You will:
Support prompt, safe hospital discharges back home (within 24 hours of referral)Empower clients to regain independence, confidence and skills through positive, safe risk-takingBuild trusting relationships and provide emotional encouragement every dayAdapt home environments to reduce risks and suit individual needsHelp clients achieve goals set by social workers with minimal ongoing supportEstablish motivating daily routines that promote great self-careAssess capabilities (physical & cognitive) over 14 days, gather evidence, and plan the right-sized care packageRefer clients to supportive tech (e.g. reminder clocks, medication dispensers, door sensors, GPS watches)Work closely with families to create person-centred care plans that everyone lovesAttend multi-agency meetings, provide progress updates, and collaborate to increase independenceWrite tailored care plans, complete risk assessments, and deliver thorough handovers to long-term teamsConnect clients with community groups and services to boost social skills and integrationTake charge of your own professional development – source relevant training and keep skills refreshed What we’re looking for?

Proven experience in domiciliary/home care (essential)A genuine person-centred approach – always putting the individual firstStrong communication skills – excellent verbal, non-verbal, and warm demeanour to connect with clients, families, and teamsCompassion, empathy, and a real passion for supporting othersProactive mindset with creative initiative and the ability to think outside the boxSolid problem-solving skills and a holistic view of care needsKnowledge of positive risk-taking and how it safely promotes independence and confidenceClear understanding of the care sector, including promoting independence and managing logistics in home settingsGreat team player – building positive relationships with clients, keyworkers, families, and colleaguesFlexible, reliable, and punctual – able to adapt to changing needs and keep things on track Here’s what makes us different:
